摘要
目的探讨益胃化瘀法对胃癌癌前病变患者的干预作用及对患者Th1/Th2平衡的影响,旨在明确中药对胃癌癌前病变患者的积极临床意义。方法将浙江大学金华医院2015年1月—2016年2月病理确诊的100例胃癌癌前病变患者随机分为对照组和治疗组。对照组采用雷贝拉唑钠肠溶片和替普瑞酮胶囊治疗,治疗组在对照组基础上同期加用益胃化瘀汤治疗,疗程12周。比较2组患者之间临床症状、病理征象和Th1/Th2细胞水平的变化情况。结果 12周治疗后,治疗组胃痛、胃胀、纳差评分分别为(1.52±1.03)分、(1.33±1.05)分、(1.38±1.01)分,明显低于对照组的(2.01±1.16)分、(2.32±1.18)分、(2.28±1.17)分,均P<0.05。治疗组炎症病理、腺体萎缩、IM、Dys的评分分别为(1.39±1.01)分、(1.37±1.03)分、(1.39±1.06)分、(1.27±1.06)分,明显低于对照组的(2.33±1.17)分、(2.51±1.18)分、(2.09±1.05)分、(2.03±1.26)分,均P<0.05。治疗组IL-2、INF-γ分别为(9.52±1.46)pg/ml、(20.37±5.20)pg/ml,明显高于对照组的(5.92±1.46)pg/ml、(14.89±4.92)pg/ml,均P<0.05;治疗组的IL-4、IL-6分别为(4.85±1.93)pg/ml、(50.43±13.51)pg/ml,明显低于对照组的(8.74±1.84)pg/ml、(80.16±11.36)pg/ml,均P<0.05。结论益胃化瘀汤不仅可有效的改善胃癌前病变患者的临床症状,并且可以调节患者的Th1/Th2平衡,改善胃镜下病理征象。
Objective To analysis the application value and Th1/Th2 immune drift of Benefit stomach blood method in the treatment of precancerous lesions of gastric cancer,clearing the positive clinical significance of TCM on precancerous lesions of gastric cancer. Methods One hundred precancerous lesions of gastric cancer patients were randomly divided into two groups,50 cases in each group. Giving rabeprazole sodium enteric-coated tablets and capsule of teprenone for control group,while above medicine and combined benefit stomach blood decoction for observe group. Then the clinical curative effect,stomachoscopy and Th1/Th2 immune index changes of the two groups were analysed. Results After 12-week treatment,the score of stomach pain/bloting/anorexia were obviously lower in treatment group（ 1. 52 ± 1. 03）,（ 1. 33 ± 1. 05）,（ 1. 38 ± 1. 01） than those in control group（ 2. 01 ± 1. 16）,（ 2. 32 ± 1. 18）,（ 2. 28 ± 1. 17）,all P〈0. 05. The score of inflammation pathological/gland atrophy/IM/Dys were obviously lower in treatment group（ 1. 39 ±1. 01）,（ 1. 37 ± 1. 03）,（ 1. 39 ± 1. 06）,（ 1. 27 ± 1. 06） than those in control group（ 2. 33 ± 1. 17）,（ 2. 51 ± 1. 18）,（ 2. 09 ± 1. 05）,（ 2. 03 ± 1. 26）,all P 0. 05.
