摘要
目的:运用网状Meta分析方法评价不同针灸疗法治疗子宫脱垂的有效性及安全性。方法:计算机检索了中国期刊全文数据库(CNKI)、万方数据库(Wanfang)、维普数据库(VIP)和中国生物医学文献数据库(SinoMed),英文数据库:PubMed、EMbase和Cochrane Library中不同针灸疗法治疗子宫脱垂的随机对照试验(randomized controlled trial,RCT),检索时间为建库至2021年11月12日。并运用Cochrane所推荐的风险偏倚评估工具对纳入文献进行质量评价以及偏倚风险评估,采用Stata 16.0以及RevMan 5.3进行统计分析。结果:共纳入文献27篇,总计1935例患者,涉及16*种干预措施,网状Meta分析结果显示,其疗效按累积排序概率图下面积(SUCRA)值由高到低排序,前6位分别为穴位中频电刺激+康复(80.8)、电针+艾灸+康复(76.3)、温针灸+康复(70.2)、艾灸+中药+康复(69.7)、穴位埋线+康复(60.2)、火针+康复(59.4),后3位分别为隔药灸+康复(26.4)、中药+康复(17.3)、康复(5.7)。结论:针灸相关疗法治疗子宫脱垂具有一定优势,优于单一康复治疗,其中穴位中频电刺激+康复最佳。
Objective:To review the clinical effectiveness and safety of different acupuncture therapies for uterine prolapse by using Network metaanalysis.Methods:The randomized controlled trials of acupuncture therapies for uterine prolapse were retrieved from the databases,starting from the date of establishment to November 12,2021.the PubMed,EMbase,Cochrane Library,CNKI,Wanfang,VIP and SinoMed.The included literature was also assessed for quality using the risk bias assessment tool recommended by Cochrane,and statistical analysis was performed using Stata 16.0 and RevMan 5.3.Result:A total of 27 literatures were included,involving 1935 patients,involving 16 intervention measures.The results of the network meta-analysis showed that,the therapeutic effect was arranged from high to the low according to the surface under cumulative ranking area(SUCRA),the top six are acupoint medium frequency electrical stimulation+rehabilitation(80.8)、electroacupuncture+moxibustion+rehabilitation(76.3),warm acupuncture+rehabilitation(70.2),moxibustion+traditional Chinese medicine+rehabilitation(69.7),acupoint catgut embedding+rehabilitation(60.2),fire acupuncture+rehabilitation(59.4).The last three are herb-separated moxibustion on navel+rehabilitation(26.4),traditional Chinese medicine+rehabilitation(17.3)and rehabilitation(5.7).Conclusion:Acupuncture therapies have superiority on treating uterine prolapse,which are better than single rehabilitation treatment,among which acupoint medium frequency electrical stimulation+rehabilitation is the best.
