电刺激生物反馈疗法联合个性化盆底康复训练对老年女性子宫脱垂患者盆底肌电值及尿动力学的影响 被引量:6

摘要 目的探究电刺激生物反馈疗法联合个性化盆底康复训练对老年女性子宫脱垂患者盆底肌电值及尿动力学的影响.方法选择本院2020年12月—2021年11月收治的96例老年女性子宫脱垂患者为研究对象,按照随机数字表法分为对照组(n=48)和观察组(n=48).对照组采用个性化盆底康复训练,观察组在对照组基础上增加电刺激生物反馈疗法,均持续干预3个月.比较两组子宫脱垂程度、盆底肌电值以及尿动力学.结果干预前,两组子宫脱垂程度、Ⅰ类、Ⅱ类肌纤维最大肌电值、各项尿动力学指标比较,组间差异无统计学意义(P>0.05);干预后,观察组子宫脱垂程度轻于对照组,Ⅰ类肌纤维最大肌电值(5.10±0.53)μV、Ⅱ类肌纤维最大肌电值(5.85±0.50)μV均高于对照组,功能性尿道长度(27.38±2.85)cm长于对照组,最大尿道闭合压(62.29±11.85)cmcmH_(2)O、腹压漏尿点压(94.89±12.60)cmH_(2)O均高于对照组,组间差异有统计学意义(P<0.05).结论临床在老年女性子宫脱垂患者中实施电刺激生物反馈疗法联合个性化盆底康复训练,能够有效减轻其子宫脱垂程度,增强盆底肌力,改善尿动力学指标. Objective To explore the effect of electrical stimulation biofeedback therapy combined with personalized pelvic floor rehabilitation training on pelvic floor electromyography and urodynamics in elderly female patients with uterine prolapse.Methods 96 elderly female patients with uterine prolapse who were admitted to the hospital from December 2020 to November 2021 were selected as the research objects,and divided into a control group(n=48)and an observation group(n=48)according to the random number table method.The control group received personalized pelvic floor rehabilitation training,and the observation group received electrical stimulation biofeedback therapy on the basis of the control group.Both groups continued to intervene for 3 months.The degree of uterine prolapse,pelvic floor electromyography and urodynamics were compared between the two groups.Results Before intervention,there was no significant difference between the two groups in the degree of uterine prolapse,the maximum EMG value of classⅠand classⅡmuscle fibers,and various urodynamic indexes(P>0.05);after the intervention,the degree of uterine prolapse in the observation group was lighter than that in the control group,the maximum EMG value of classⅠmuscle fibers was(5.10±0.53)μV and the maximum EMG value of classⅡmuscle fibers was(5.85±0.50)μV,which were higher than those of the control group,the length of functional urethra was(27.38±2.85)cm,which was longer than that of the control group,the maximum closed urethral pressure was(62.29±11.85)cmcmH_(2)O,and the abdominal pressure leaking point pressure was(94.89±12.60)cmH_(2)O,which were higher than those of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Clinical application of electrical stimulation biofeedback therapy combined with personalized pelvic floor rehabilitation training in elderly women with uterine prolapse can effectively reduce the degree of uterine prolapse,enhance pelvic floor muscle strength,and improve urodynamic indicators.
