摘要
目的探讨背带式子宫缝合配合宫腔内水囊压迫在剖宫产术中难治性产后出血治疗中的应用效果。方法选取广东省惠州市仲恺高新区人民医院2017年1月至2020年7月接收的83例剖宫产术中难治性产后出血患者临床资料,依据随机数字表法分组,对照组(40例)采取宫腔水囊压迫治疗,观察组(43例)接受背带式子宫缝合配合宫腔内水囊压迫治疗,比较两组出血量、手术时间、止血效果、切除子宫状况、术后预后状况。结果切除子宫率、止血成功率、子宫复旧率、产褥感染发生率方面,观察组分别是2.33%、100.00%、95.34%、4.65%,对照组分别是15.00%、90.00%、70.00%、20.00%,观察组均优于对照组,差异有统计学意义(P<0.05);术中出血量、手术时间方面,观察组均比对照组少,差异有统计学意义(P<0.05)。结论背带式子宫缝合联合宫腔水囊压迫止血在剖宫产术中难治性产后出血治疗中应用效果明显,除减少手术时间、术中出血量外,有助于患者预后改善,值得采纳与推广。
Objective To investigate the application efficacy of strap-type uterine suture combined with intrauterine water sac compression on patients with intractable postpartum hemorrhage during cesarean section.Methods The clinical data of 83 patients with intractable postpartum hemorrhage during cesarean section admitted to the People’s Hospital of Huizhou Zhongkai Hi-tech Industrial Development Zone from January 2017 to July 2020 were selected and divided into the control group(n=40)and the observation group(n=43)according to the random number table method.The control group was treated with intrauterine water sac compression,while the observation group was treated with strap-type uterine suture combined with intrauterine water sac compression.The hemorrhage volume,operation duration,hemostatic efficacy,hysterectomy status and postoperative prognosis of the two groups were compared.Results The rates of hysterectomy,hemostasis success,uterine involution and puerperal infection in the observation group were 2.33%,100.00%,95.34%and 4.65%,superior to 15.00%,90.00%,70.00%and 20.00%in the control group respectively,with statistically significant differences(P<0.05).In terms of intraoperative hemorrhage volume and operation duration,the observation group was less than the control group,with statistically significant differences(P<0.05).Conclusion Strap-type uterine suture combined with intrauterine water sac compression hemostasis is effective in the treatment of intractable postpartum hemorrhage during cesarean section,which not only reduces the operation duration and the intraoperative hemorrhage volume,but also helps to improve the prognosis of patients.Therefore,it is worthy of adoption and promotion.
