摘要
目的观察对剖宫产术中发生难治性产后出血的患者实施双侧子宫动脉上行支结扎联合改良式子宫背带式缝合术的止血效果。方法将70例剖宫产术中发生难治性产后出血的产妇随机分为对照组和观察组各35例,对照组实施双侧子宫动脉上行支结扎术,观察组实施双侧子宫动脉上行支结扎术联合改良式子宫背带式缝合术。对比2组手术时间、术中出血量、术中输血量、术后24h内输血量、子宫切除率以及产褥热发生率的差异。结果2组手术时间差异无统计学意义(P>0.05);观察组术中出血量、术中输血量和术后24h输血量均显著少于对照组,差异有统计学意义(P<0.05);2组子宫切除率差异无统计学意义(P>0.05);观察组产褥热发生率显著低于对照组,差异有统计学意义(P<0.05)。结论与双侧子宫动脉上行支结扎术相比,子宫动脉上行支结扎联合改良式子宫背带式缝合术可以显著减少产后出血量、输血量及产褥热的发生率。
Objective To investigate the effect of bilateral uterine artery ascending branch ligation+modified uterine brace suture in intractable postpartum hemorrhage during cesarean delivery.Methods Seventy casees of puerpera with intractable postpartum hemorrhage during cesarean delivery were randomly divided into control group and observation group,with 35 cases in each group.Puerperas in the control group were treated with bilateral uterine artery ascending branch ligation,while puerperas in the observation group were treated with bilateral uterine artery ascending branch ligation+modified uterine brace suture.Surgery time,intraoperative bleeding volume,intraoperative transfusion volume,transfusion volume within 24 hours postoperatively,the rate of hysterectomy and the rate of puerperal fever were compared between the two groups.Results There was no significant difference in surgery time between the two groups(P>0.05).The intraoperative bleeding volume,intraoperative transfusion volume,transfusion volume within 24 hours postoperatively in the observation group were significantly less than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the rate of hysterectomy between the two groups(P>0.05).The rate of puerperal fever in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Compared with bilateral uterine artery ascending branch ligation,bilateral uterine artery ascending branch ligation+modified uterine brace suture can significantly reduce the postpartum hemorrhage,blood transfusion volume postoperatively and the incidence of puerperal fever.
