摘要
目的:分析门诊药房退药原因,促进临床合理用药。方法:将退药原因归纳为9类,运用管理学上的帕累托图对本院门诊药房退药处方进行分析。结果:造成患者退药的前3项因素(ADR、不合理用药、医生笔误或收费错误)为主要因素,患者病情变化、医务人员因素为次要因素,其余为一般因素。结论:应针对存在的问题制定诸如开展临床药学服务、提高医务人员的服务意识和业务学习、提高服务质量等措施,本院药事管理委员会应制定出台相应的奖惩措施条款以降低退药率。
Objective： To analyze the reasons for repercussion of Western medicine in outpatient dispensary to promote reasonable clinical medication. Method：The reasons for the drug repercussion were classified into 9 kinds; pareto diagram in principle of management was used to analyze the drug repercussion prescriptions about the outpatient dispensary for Western medicine in our hospital. Result：The reasons of ADR, non-rational drug use and clerical errors of doctors or mistakes of cashiers for drug repercussion were the major factors, patient＇ s progress of disease and factors of medical workers took up the second place, and the others were the general considerations. Conclusion：Some effective measures should be taken： such as carrying out pharmaceutical care in outpatient dispensary, enhancing medical workers, consciousness of service and professional level, improving medical service quality and so on, but the most important is the Pharmaceutical Management Committee of our hospital should should work out some relevant regulations and measures to reduce the rate of drug repercussion.
