摘要
目的:了解北京某医院2019年门诊不合理处方的发生原因,并进行帕累托图分析,以期为临床合理用药提供参考。方法:按1%的比例随机抽取该院门诊处方,对其进行合理性评价,并用帕累托图对不合理处方存在问题类型进行分析。结果:共抽取门诊处方18206张,其中不合理处方1358张,占总点评处方数的7.46%。该院不合理处方存在11类问题,主要因素2项,次要因素2项,一般因素7项,其中适应证不适宜、处方超量、用法与用量不适宜以及重复用药是导致该院门诊处方不合理的主要因素。结论:应用帕累托图分析可简单、直观地反应出该院门诊处方不合理的主要因素,临床应规范医师的处方行为,医院应积极推进处方前置审核工作,保证临床合理用药。
OBJECTIVE:To investigate the causes of irrational outpatient prescriptions in a hospital in Beijing in 2019,and to perform Pareto chart analysis,so as to provide reference for clinical rational drug use.METHODS:The outpatient prescriptions were randomly sampled for rationality review at a rate of 1%.The types of problems in the irrational prescriptions were analyzed by Pareto chart.RESULTS:A total of 18206 outpatient prescriptions were selected,including 1358(7.46%)irrational prescriptions.There were 11 kinds of irrational prescriptions in this hospital,including 2 main factors,2 secondary factors and 7 general factors.Inappropriate indications,excessive prescriptions,improper usage and dosage and repeated medication were the main factors that lead to irrational prescriptions in the outpatient department of the hospital.CONCLUSIONS:The application of Pareto chart analysis can simply and intuitively reflect the main factors of irrational outpatient prescription in the hospital,prescription behavior of clinicians should be standardized clinically,and the hospital should actively promote the pre-review of prescriptions to ensure rational clinical drug use.
