摘要
本文主要介绍郭志强治疗围绝经期女性的经验。郭志强认为,围绝经期女性带下减少,肝肾渐亏,阴血亏虚,生风化燥,或湿瘀邪毒下注,阻滞气血运行,致使阴部皮肤失养而瘙痒不宁。归纳其主要发病原因为,阴血不足兼有湿瘀邪毒外侵;辨证分为血虚化燥、肝肾阴虚、脾肾阳虚、肝郁化热、湿热下注等证型,其中以“肝肾阴虚证”最为多见。治以养阴润燥、养血祛风为主,以健脾清热利湿、活血化瘀为辅;阴损及阳时,则需阴阳双补;治疗过程中将治病必顾护脾胃的思想贯彻始终。临床上根据不同证型采用中药内服、外敷、外洗、维生素制剂外抹等措施综合治疗,同时重视心理治疗,临床疗效显著。
The article introduces the experience of professor GUO Zhiqiang in treating non-neoplastic epithelial lesions of the vulva in menopausal women.GUO Zhiqiang believes that menopausal women experience a decrease in vaginal discharge,gradual deficiency of liver and kidney,as well as deficiency of yin and blood,leading to the generation of wind and dryness,or the downward flow of dampness,stagnation,and pathogenic toxins that obstruct the circulation of qi and blood,resulting in malnourishment and itching of the genital skin.The main etiology is summarized as insufficiency of yin-blood accompanied by external invasion of dampness,stagnation,and pathogenic toxins.The syndrome differentiation mainly includes blood deficiency leading to dryness,yin deficiency of liver and kidney,yang deficiency of spleen and kidney,liver depression transforming into heat,and damp-heat pouring downward,among which"yin deficiency of liver and kidney"is the most common one.The treating approach is to nourish yin and moisturize dryness,nourish blood and dispel wind,supplemented by strengthening the spleen,clearing heat,and promoting blood circulation to resolve stasis.When yin deficiency affects yang,it is necessary to replenish both yin and yang.In the process of treatment,the thought of protecting the spleen and stomach is always carried out.Clinically,according to different syndrome types,comprehensive treatment measures such as Chinese medicine in internal and external application,external washing and vitamin preparation are adopted.At the same time,we also pay attention to psychological treatment,which has remarkable curative effect.
