摘要
目的探讨大分割调强放疗(IMRT)在乳腺癌保乳术后的应用效果及安全性。方法队列研究。抽取2019年4月至2020年2月于焦作市第二人民医院接受乳腺癌保乳手术并行放疗的患者100例,按照随机数字表法分为对照组和研究组,每组50例。对照组采用IMRT下常规分割方案,研究组采用IMRT下大分割方案。比较两组患者的美容效果、放疗相关不良反应、放疗费用与住院时长、焦虑自评量表(SAS)评分和生存率等指标。结果研究组患者美容效果满意率(98.00%,49/50)与对照组(96.00%,48/50)比较,差异未见统计学意义(P>0.05);研究组放疗相关不良反应发生情况及1、2、3年生存率与对照组比较,差异均未见统计学意义(P均>0.05);研究组放疗费用与住院时长均少于对照组(P均<0.05);两组患者放疗结束时SAS评分均较放疗前降低,且研究组低于对照组(P<0.05)。结论调强放疗下大分割方案在乳腺癌保乳术后放疗中具有较好的疗效和安全性,可以有效提高治疗效果。
Objective To investigate the effect and safety of hyperfractionated intensity-modulated radiotherapy(IMRT)after breast conserving surgery for breast carcinoma.Methods A cohort study was adopted in this study.A total of 100 patients who received breast conserving surgery and radiotherapy for breast carcinoma in the Second People’s Hospital of Jiaozuo from April 2019 to February 2020 were selected,and they were divided into a control group and a study group using a random number table method,with 50 cases in each group.The control group was treated by conventional fractionation scheme IMRT,while the study group was treated by hyperfractionation scheme IMRT.The cosmetic effects,radiotherapy related adverse reactions,radiotherapy costs and length of hospital stay,self rating anxiety scale(SAS)scores,and survival rates of the two groups were compared.Results There was no significant difference in satisfaction rate of cosmetic effects between the study group(98.00%,49/50)and the control group(96.00%,48/50),P>0.05.There was no statistically significant difference in the incidence of radiotherapy related adverse reactions and the 1-,2-,3-year survival rate between the study group and the control group(all P>0.05).The cost of radiotherapy and length of hospital stay in the study group were lower than those in the control group(all P<0.05).At the end of radiotherapy,the SAS scores of both groups decreased compared with the scores before radiotherapy,and the study group had lower SAS scores than the control group(P<0.05).Conclusions Hyperfractionated intensity-modulated radiotherapy has great efficacy and safety in radiotherapy after breast conserving surgery for breast carcinoma,which can effectively improve the therapeutic effect.
