乳腺癌保乳术后调强放疗在计算机断层扫描图像引导下的摆位误差分析 被引量:3

Discussion on Position Error of Intensity-Modulated Radiation Therapy After Breast-Conserving Surgery for Breast Cancer Guided by Computed Tomography

摘要 目的探讨乳腺癌保乳术后调强放疗在计算机断层扫描(CT)图像引导下的摆位误差。方法收集2015年1月至2018年10月我院收治的经病理学确诊的120例乳腺癌保乳术后调强放疗患者的临床资料。根据放疗计划,将120患者摆放好体位,用专门的辅助固定系统进行固定。根据放疗计划的参考数据,进行CT图像选择中心点,然后进行正侧位拍片,和计划图像进行对比,计算摆位误差。观察第1、5、10次放疗摆位误差情况。结果120例患者第1、5、10次放疗时在X、Y、Z轴3个方向摆位误差均低于5mm,而且Z轴方向误差最小,与X、Y轴比较差异均有统计学意义(P均<0.05)。结论乳腺癌保乳术后调强放疗时,CT图像引导下的摆位误差明显缩小,应用价值较高,值得临床推广。 Objective To investigate the position error of intensity-modulated radiation therapy after breast-conserving surgery for breast cancer guided by computed tomography (CT) image. Methods The clinical data of 120 patients with breast cancer after breast-conserving surgery were collected from January 2015 to October 2018.According to the regulations, all the patients were placed in a good position and fixed with a special auxiliary fixation system.According to the reference data of the radiotherapy plan, the CT image selection center point was performed, and then the film was taken, and the position error was calculated through comparing with the planned image.The first, fifth, and tenth radiotherapy position errors were observed. Results The first, fifth and tenth radiotherapy of the patients were less than 5 mm in the X, Y and Z axis directions, and the error in the Z axis direction was the smallest, compared with the X axis and the Y axis directions ( P <0.05). Conclusion In the intensity-modulated radiation therapy after breast-conserving surgery for breast cancer, CT image guidance can reduce the position error and has the higher application value.
