摘要
目的评价以问题为基础的学习(problem-based learning,PBL)结合循证医学(evidence-based medicine,EBM)教学模式在住院医师规范化培训(简称住培)中的应用效果。方法计算机检索知网、万方、维普数据库中有关PBL结合EBM(PBL-EBM)教学模式对比传统讲授式教学模式(lecture-based learning,LBL)在住培中应用效果的研究,检索时限从建库至2022年7月31日。采用RevMan 5.3软件对纳入研究进行Meta分析。结果最终共纳入研究文献17篇,包含研究对象1036名,其中试验组520名,对照组516名。结果显示,与LBL相比,应用PBL-EBM教学模式不仅提高了住培学员理论考试(SMD=1.53,95%CI:0.96~2.11)和操作考试(SMD=1.93,95%CI:1.11~2.75)的成绩,还提高了个人表达能力(RR=1.97,95%CI:1.43~2.73)、团队协作能力(RR=1.91,95%CI:1.46~2.49)、发现与解决问题能力(RR=1.61,95%CI:1.42~1.82)、临床思维能力(RR=1.51,95%CI:1.33~1.71)和自学能力(RR=2.44,95%CI:1.98~3.00),差异有统计学意义(P<0.05)。结论与LBL教学法相比,应用PBL-EBM教学法能够提高住培学员的培训效果。
Objective To evaluate the effect of problembased learning(PBL)combined with the evidence-based medicine(EBM)teaching method in standardized residency training.Methods The databases of CNKI,Wanfang and VIP were searched for studies on the application effect of PBL-EBM teaching mode compared with traditional lecture-based learning(LBL)teaching mode in the standardized training of residents.The search was done by July 31,2022.The paper used RevMan 5.3 software to perform a meta-analysis of the included studies.Results In total,17 studies were included in the review,which involved 1036 participants,while 520 participants were in PBL-EBM group,516 were in the LBL group.The meta-analysis showed that compared to the LBL group,the PBL-EBM method not only significantly elevated the theoretical(SMD=1.53,95%CI:0.96~2.11)and operational(SMD=1.93,95%CI:1.11~2.75)examination score,but also improved the abilities of personal expression(RR=1.97,95%CI:1.43~2.73),team cooperation(RR=1.91,95%CI:1.46~2.49),problem finding and solving(RR=1.61,95%CI:1.42~1.82),clinical thinking(RR=1.51,95%CI:1.33~1.71),and self-learning(RR=2.44,95%CI:1.98~3.00).Conclusion Current evidence suggests that the application of PBL combined with EBM teaching methods can improve the pedagogical impact of resident standardization training.
