摘要
多囊卵巢综合征患者常伴有胰岛素抵抗(IR),胰岛素的不断上升会刺激卵巢分泌过多的雄激素,从而导致下丘脑-垂体-卵巢轴出现异常,进而引起排卵障碍,造成不孕。不孕往往是造成育龄期女性压力大、抑郁的主要原因之一,影响着她们的生活质量。因此,改善PCOS-IR患者的排卵障碍,恢复规律的排卵在PCOS治疗中具有重要意义。多囊卵巢综合征所致排卵障碍是妇科常见的疑难杂症,其病理生理的确切机制尚不明确,亟待大量研究加以证实,文章从排卵障碍角度研究多囊卵巢综合征,为更深刻认识多囊卵巢综合征的病理生理提供了新的角度。中医、西医对多囊卵巢综合征伴胰岛素抵抗排卵障碍的治疗各有所长,明确不同疗法在治疗中的意义,有助于指导未来PCOS的治疗工作,帮助患者提高妊娠率。
Patients with polycystic ovary syndrome(PCOS)often have insulin resistance(IR).The rising insulin stimulates the ovaries to secrete too much androgen,which leads to abnormalities in the hypothalamic-pituitary-ovarian axis,which in turn causes ovulation disorders,resulting in infertility.Infertility is often one of the main causes of stress and depression in women of childbearing age.It affects their quality of life.Therefore,improving ovulation disorders and restoring regular ovulation in patients with PCOS-IR is of great importance in the treatment of PCOS.The exact mechanism of its pathophysiology is still unclear and needs to be confirmed by numerous studies.This paper studies polycystic ovary syndrome from the perspective of ovulation disorder,which provides a new perspective for a deeper understanding of the pathophysiology of polycystic ovary syndrome.The treatment of ovulation disorders in polycystic ovary syndrome with insulin resistance by TCM and Western medicine has its own strengths,and clarifying the significance of different therapies in the treatment will help guide the future treatment of PCOS and help patients improve their pregnancy rate.
