摘要
目的探讨多囊卵巢综合征(PCOS)不孕症患者血清抗苗勒管激素(AMH)、抑制素B(INHB)、25-羟维生素D[25-(OH)D]的变化及其与性激素的相关性。方法采用回顾性方法,收集2018年1月-2020年12月期间银川国龙骨科医院收治的PCOS不孕症患者108例作为研究组,同期收集本院体检中心女性健康体检者100名作为对照组。检测2组研究对象的AMH、INHB、25-(OH)D以及性激素表达水平,并分析其与卵泡刺激素(FSH)、黄体生成素(LH)、睾酮(T)的相关性。结果研究组AMH、INHB、FSH、LH、T水平分别为(13.11±1.62)ng/mL、(153.76±21.02)pg/mL、(6.13±0.72)IU/L、(11.33±2.46)IU/L、(0.89±0.18)ng/mL,明显高于对照组[(2.88±0.56)ng/mL、(100.36±23.14)pg/mL、(5.55±0.68)IU/L、(5.03±0.78)IU/L、(0.40±0.11)ng/mL],25-(OH)D水平(17.88±6.72)ng/mL明显低于对照组(33.46±8.81)ng/mL,差异均有统计学意义(t=60.347、12.459、11.435、20.344、14.243、23.471,P均<0.05)。AMH、INHB、25-(OH)D联合诊断PCOS不孕症的曲线下面积(AUC)为0.918,高于AMH、INHB、25-(OH)D单独检测的0.885、0.810、0.696;灵敏度、特异度分别为90.0%、84.8%。Pearson相关性分析显示,AMH、INHB与FSH、LH、T均成正相关(r=0.333、0.308、0.154,0.305、0.123、0.147;P均<0.05);25-(OH)D与FSH、LH、T无明显相关性(r=0.159、0.265、0.198,P均>0.05)。结论PCOS不孕症患者血清AMH、INHB水平显著升高,25-(OH)D水平显著降低,且AMH、INHB与性激素水平明显相关,测定血清AMH、INHB、25-(OH)D水平有助于PCOS的诊断。
Objective To investigate the changes of serum anti-mullerian hormone(AMH),inhibin B(INHB),25-hydroxyvitamin D[25-(OH)D]and their correlation with sex hormones in infertile patients with polycystic ovary syndrome(PCOS).Methods Retrospectively,the cases of PCOS infertility patients admitted to Yinchuan Guolong Orthopedics Hospital from January 2018 to December 2020 were collected as the study group,and 100 cases of female health examinees in the physical examination center of our hospital were collected as the control group during the same period.The expression levels of AMH,INHB,25-(OH)D and sex hormones in the two groups were detected,and the correlations between them and follicle stimulating hormone(FSH),luteinizing hormone(LH)and testosterone(T)were analyzed.Results The levels of AMH,INHB,FSH,LH and T in the study group were(13.11±1.62)ng/mL,(153.76±21.02)pg/mL,(6.13±0.72)IU/L,(11.33±2.46)IU/L and(0.89±0.18)ng/mL,respectively,which were obviously higher than those in the control group[(2.88±0.56)ng/mL,(100.36±23.14)pg/mL,(5.55±0.68)IU/L,(5.03±0.78)IU/L,(0.40±0.11)ng/mL],and the level of 25-(OH)D was(17.88±6.72)ng/mL,which was obviously lower than that in the control group(33.46±8.81)ng/mL,the differences were statistically significant(t=60.347,12.459,11.435,20.344,14.243,23.471;P all<0.05).The area under curve(AUC)of AMH,INHB and 25-(OH)D combined diagnosis of PCOS was 0.918,which was higher than AMH,INHB and 25-(OH)D alone(0.885,0.810,0.696);the sensitivity and specificity were 90.0%and 84.8%,respectively.Pearson correlation analysis showed that AMH and INHB were positively correlated with FSH,LH,and T(r=0.333,0.308,0.154;0.305,0.123,0.147;P all<0.05);and 25-(OH)D had no significant correlation with FSH,LH,and T(r=0.159,0.265,0.198,P all>0.05).Conclusions Serum levels of AMH and INHB in infertile patients with PCOS were significantly increased,and 25-(OH)D was significantly decreased,which was significantly related to the levels of AMH and INHB.The determinations of serum levels of AMH,INHB and 25-(OH)D were helpful for the diagnosis of PCOS.
