多囊卵巢综合征患者血清和肽素水平与糖脂代谢及IR的关系研究 被引量:13

Relationship of Serum Copeptin Level with Glucose and Lipid Metabolism and IR in Patients with Polycystic Ovary Syndrome

摘要 目的探究多囊卵巢综合征(PCOS)患者血清和肽素水平与糖脂代谢及胰岛素抵抗(IR)的关系。方法选取2019年1—12月于妇科就诊的80例PCOS患者为研究组,选取同期进行健康体检的80例育龄期妇女为对照组。研究组每日口服二甲双胍,连续治疗6个月。收集2组临床资料,测定收缩压(SBP)、舒张压(DBP)、黄体生成素(LH)、促性腺激素(FSH)、睾酮(T)、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、空腹血糖(FPG)、餐后2 h血糖(2 h PG)、空腹胰岛素(FINS)、糖化血红蛋白(HbA1c)水平,计算稳态模型胰岛β细胞功能指数(HOMA-β)和稳态模型胰岛素抵抗指数(HOMA-IR),采用酶联免疫吸附法(ELISA)检测血清和肽素水平。结果研究组体重和体重指数(BMI)显著高于对照组(P<0.05);研究组SBP、DBP、LH、T、TG、FPG、2 h PG、FINS、HOMA-β、HOMA-IR及血清和肽素水平显著高于对照组,HDL-C水平显著低于对照组(P<0.05,P<0.01)。PCOS患者血清和肽素水平与体重、BMI、TG、FPG、2 h PG、FINS、HOMA-β、HOMA-IR呈正相关,与HDL-C呈负相关(P<0.05,P<0.01)。与治疗前比较,治疗后PCOS患者SBP、DBP、LH、T、TG、FPG、2 h PG、FINS、HOMA-β、HOMA-IR及血清和肽素水平均显著降低,HDL-C水平显著提高(P<0.05,P<0.01)。结论和肽素可能参与PCOS病理生理过程,其水平与糖脂代谢、IR及心血管疾病发生风险相关。二甲双胍可降低PCOS患者血清和肽素水平。 Objective To explore the relationship of serum copeptin level with glucose and lipid metabolism and IR in patients with polycystic ovary syndrome(PCOS).Methods A total of 80 patients with PCOS treated in Gynecology Department of our hospital between January 2019 and December 2019 were selected as the research group,and women of childbearing age who completed physical checkups in our hospital during the same period were selected as the control group.The research group was given oral Metformin daily for 6 months.Clinical data of all subjects were collected,and systolic blood pressure(SBP),diastolic blood pressure(DBP),luteinizing hormone(LH),gonadotropin(FSH),testosterone(T),triacylglycerol(TG),total cholesterol(TC),low density lipoprotein(LDL-C),high density lipoprotein(HDL-C),fasting plasma glucose(FPG),postprandial 2 h plasma glucose(2 h PG),fasting insulin(FINS)and glycosylated hemoglobin(HbA1c)levels were measured.The pancreaticβ-cells functionindex(HOMA-β)and insulin resistance index(HOMA-IR)were calculated.Enzyme-linked immunosorbent assay(ELISA)was used to detect serum copeptin levels.Results Weight and BMI of the research group were significantly higher than those of the control group(P<0.05).SBP,DBP,LH,T,TG,FPG,2 h PG,FINS,HOMA-β,HOMA-IR and serum copeptin levels in the research group were significantly higher than those in the control group,while HDL-C levels were significantly lower than those in the control group(P<0.05,P<0.01).Serum copeptin levels in patients with PCOS were significantly and positively correlated with their body weight,BMI,TG,FPG,2 h PG,FINS,HOMA-βand HOMA-IR but were significantly and negatively correlated with HDL-C(P<0.05,P<0.01).After treatment,SBP,DBP,LH,T,TG,FPG,2 h PG,FINS,HOMA-β,HOMA-IR and serum copeptin levels in patients with PCOS were significantly decreased,while HDL-C levels were significantly increased(P<0.05,P<0.01).Conclusion Serum copeptin may be involved in the pathophysiological process of PCOS,and its level is related to metabolism,insulin resistance and the risk of cardiovascular diseases.Treatment with Metformin can significantly reduce serum copeptin levels in patients with PCOS.
