摘要
目的:观察补肾通络方联合降糖药物对2型糖尿病性骨质疏松症肾虚血瘀证患者骨密度、钙磷代谢及血液流变学指标的影响。方法:将80例2型糖尿病性骨质疏松症肾虚血瘀证患者随机分为治疗组(40例)和对照组(40例)。对照组患者给予常规临床治疗,治疗组患者在对照组基础上联合补肾通络方治疗。治疗3个月后,比较两组患者临床疗效、骨密度、尿钙、尿磷、血糖指标[空腹血糖(FBG)、餐后2 h血糖(2 hPG)、糖化血红蛋白(HbA1c)]、血液流变学指标(全血高切黏度、全血低切黏度、红细胞刚性指数、红细胞聚集指数)及药物安全性。结果:治疗组总有效率为90.00%(36/40),对照组总有效率为72.50%(29/40),治疗组优于对照组(P<0.05);两组患者治疗后骨密度均升高(P<0.05),且治疗组患者治疗后骨密度高于对照组(P<0.05);两组患者治疗后尿钙和尿磷均降低(P<0.05),且治疗组患者治疗后尿钙和尿磷均低于对照组(P<0.05);两组患者治疗后FBG、2 hPG及HbA1c水平均降低(P<0.05),且治疗组患者治疗后FBG、2 hPG及HbA1c水平均低于对照组(P<0.05);两组患者治疗后全血高切黏度、全血低切黏度、红细胞刚性指数及红细胞聚集指数均降低(P<0.05),且治疗组患者治疗后全血高切黏度、全血低切黏度、红细胞刚性指数及红细胞聚集指数均低于对照组(P<0.05);两组患者治疗期间均未见药物相关不良反应。结论:补肾通络方联合降糖药物能改善2型糖尿病性骨质疏松症肾虚血瘀证患者血液流变学指标,调节钙磷代谢和血糖指标,有助于提高患者骨密度,疗效确切,安全可靠。
Objective:To observe the effect of Bushen Tongluo recipe combined with hypoglycemic drugs on bone mineral density,calcium and phosphorus metabolism and hemorheological indexes in patients with type 2 diabetic osteoporosis and kidney deficiency and blood stasis syndrome.Methods:A total of 80 patients with type 2 diabetic osteoporosis and kidney deficiency and blood stasis syndrome were randomly divided into treatment group(40 cases)and control group(40 cases).The patients in the control group were given routine clinical treatment,and the patients in the treatment group were treated with Bushen Tongluo recipe on the basis of the control group.After 3 months of treatment,the clinical efficacy,the density of bone mineral,the levels of urinary calcium and urinary phosphorus,blood glucose indexes[fasting blood glucose(FBG),2 h postprandial blood glucose(2 HPG),glycosylated hemoglobin(HbA1c)],hemorheological indexes(whole blood high shear viscosity,whole blood low shear viscosity,erythrocyte rigidity index,erythrocyte aggregation index)and drug safety between the two groups were compared.Results:The total effective rate was 90.00%(36/40)in the treatment group and 72.50%(29/40)in the control group.The treatment group was better than the control group(P<0.05).The bone mineral density of the two groups increased after treatment(P<0.05),and the bone mineral density of the treatment group was higher than that of the control group(P<0.05).The urinary calcium and phosphorus levels in the two groups of patients decreased after treatment(P<0.05),and the urinary calcium and phosphorus levels of patients in the treatment group were lower than those in the control group(P<0.05).The levels of FBG,2 hPG and HbA1c in the two groups of patients decreased after treatment(P<0.05),and the levels of FBG,2 hPG and HbA1c in the treatment group were lower than those in the control group(P<0.05).After treatment of the two groups of patients,the whole blood high shear viscosity,whole blood low shear viscosity,red blood cell stiffness index and red blood cell aggregation index in the two groups of patients decreased after treatment(P<0.05),and the whole blood high shear viscosity,whole blood low shear viscosity,erythrocyte rigidity index and erythrocyte aggregation index in the treatment group were lower than those in the control group(P<0.05).There were no drug-related adverse reactions in both groups during treatment.Conclusion:Bushen Tongluo recipe combined with hypoglycemic drugs can improve the hemorheology indexes of patients with kidney deficiency and blood stasis syndrome in type 2 diabetic osteoporosis,regulate calcium and phosphorus metabolism and blood glucose indexes,which is helpful to improve bone mineral density,and is effective,safe and reliable.
