摘要
中医学对糖尿病心脏病病机的认识在不断发展与变化。在梳理糖尿病心脏病中医病机认识的历史源流和现代发展成果的基础上,认为糖尿病"虚""热""瘀"的病变特点是糖尿病心脏病的病机基础;气阴两虚、痰瘀互结、心络痹阻是基本病机,其中气阴两虚是形成糖尿病心脏病的始动因素,痰瘀互结是糖尿病心脏病发生发展的关键环节,心络痹阻是糖尿病心脏病的表现形式;阴火病机、少阴枢机病机、伏邪病机、浊邪病机等经典病机理论对糖尿病心脏病防治具有重要意义。从中医病机从发,糖尿病心脏病防治关口前移、个体化疗效评价方法的建立、病证结合研究新模式的构建等科学问题仍需要进一步探索。
The understanding of the pathogenesis of diabetic heart disease(DHD)in traditional Chinese medicine(TCM)is changing constantly.Through analyzing the historical origin and the modern achievements,it is considered that the pathological characteristics of"deficiency","heat"and"stasis"in diabetes mellitus are the basis for interpreting the pathogenesis of DHD.Deficiency of qi and yin,binding of phlegm and stasis and heart collateral obstruction are the basic pathogenesis,of which deficiency of qi and yin is the initiating factor for the formation of DHD;binding of phlegm and stasis is closely related to the development of the disease;and heart collateral obstruction is the manifestation.The classical pathogenesis theories,such as"yin fire","shaoyin(少阴)pivot","latent pathogen"and"turbid pathogen",are of great significance to the prevention and treatment of DHD.From the TCM pathogenesis,certain scientific issues need to be further explored,including the moving-forward of threshold for DHD prevention,the establishment of individualized therapeutic effect evaluation methods,and the construction of a new research model to combine the disease and syndrome.
