参麦注射液辅助治疗冠心病临床研究 被引量:6

Clinical Effect of Shenmai Injection(参麦注射液)in Treatment of Coronary Heart Disease

摘要 目的分析参麦注射液辅助治疗对冠心病患者心功能及血液黏度的影响。方法选择2020年1月—2021年4月天津市职业病防治院收治的冠心病患者80例,随机分为对照组及观察组,对照组给予欣康口服等常规综合治疗,观察组在对照组治疗的基础上给予参麦注射液治疗。观察并比较两组患者治疗前后临床症状发生情况及治疗效果。分别于治疗前后采集两组患者的空腹静脉血,采用血液流变分析仪检测全血高切还原黏度、全血低切还原黏度、血浆黏度、红细胞聚集指数、红细胞刚性指数、红细胞变形指数水平,采用全自动凝血分析仪检测纤维蛋白原(Fg),采用全自动血细胞分析仪检测血细胞比容(Hct)水平,采用时间分辨免疫荧光分析法检测全血心肌肌钙蛋白I(cTnI)及氨基末端B型钠尿肽前体(NT-proBNP)水平。分别于治疗前后采用多普勒超声心动图仪检测两组患者射血分数(EF)水平。观察并比较两组患者治疗期间并发症、不良反应发生情况。结果观察组患者治疗总有效率为92.50%(37/40),明显高于对照组的75.00%(30/40)(P<0.05)。治疗后两组患者下肢水肿、心悸气促、肝颈静脉回流征阳性、颈静脉怒张、肝大及乏力等临床症状发病率明显低于同组治疗前,其中治疗后观察组患者临床症状发生率明显低于对照组(P<0.05)。治疗后两组患者ET水平明显高于同组治疗前,cTnI及NT-proBNP水平明显低于同组治疗前,其中治疗后观察组患者ET水平明显高于对照组,cTnI及NT-proBNP水平明显低于对照组(P<0.05)。治疗后两组患者全血高切还原黏度、全血低切还原黏度、血浆黏度、红细胞聚集指数、红细胞刚性指数、红细胞变形指数、Fg及Hct水平明显低于同组治疗前,其中治疗后观察组患者血高切还原黏度、全血低切还原黏度、血浆黏度、红细胞聚集指数、红细胞刚性指数、红细胞变形指数、Fg及Hct水平水平明显低于对照组(P<0.05)。观察组患者并发症发生率为5.00%(2/40),明显低于对照组的20.00%(8/40),观察组患者不良反应发生率为7.50%(3/40),明显低于对照组的25.00%(10/40)(P<0.05)。结论参麦注射液辅助治疗冠心病的效果显著,可明显减轻患者症状,提高患者心功能,改善患者血流功能状态,且安全可靠,可在临床中广泛应用。 Objective To analyze the effect of Shenmai Injection(参麦注射液)on the heart function and blood viscosity of coronary heart disease patients.Methods Eighty patients with coronary heart disease who were treated in the hospital from January 2020 to April 2021 were randomly divided into the control group and the observation group.The control group was given Xinkang orally and other conventional comprehensive treatment.The observation group was treated with Shenmai Injection on the basis of the treatment of the control group.The clinical symptoms and therapeutic effect of the two groups were observed and compared.The fasting vena blood of the two groups was collected before and after treatment.The high cut reduction viscosity,low cut reduction viscosity,plasma viscosity,RBC aggregation index,red cell rigidity index and red blood cell deformation index were measured by Hemorheology analyzer,and fibrinogen(Fg)was detected by automatic coagulation analyzer.The level of HCT was detected by the automatic blood cell analyzer,and the levels of cardiac troponin I(cTnI)and NT proBNP were detected by time-resolved immunofluorescence analysis.The level of EF was measured by Doppler echocardiography before and after treatment.The complications and adverse reactions were observed and compared between the two groups.Results The total effective rate in the observation group was 92.50%(37/40),which was significantly higher than that(75.00%,30/40)of the control group(P<0.05).After treatment,the incidence of clinical symptoms such as edema,palpitation,shortness of breath,positive hepatic vein and neck vein,neck vein congestion,liver and fatigue was significantly lower in the two groups.The incidence of clinical symptoms in the observation group was significantly lower than that in the control group(P<0.05).After treatment,ET levels of the two groups were significantly higher than those before treatment,and the levels of cTnI and NT proBNP were significantly lower than those before treatment,among which the ET levels of the observation group were significantly higher than those in the control group,and the levels of cTnI and NT proBNP were significantly lower than those in the control group(P<0.05).After treatment,the blood viscosity of the two groups was significantly lower than that of the same group,including high cut reduction viscosity,low cut reduction viscosity,plasma viscosity,RBC aggregation index,red cell rigidity index,red cell deformation index,levels of Fg and HCT in the two groups,among which,the blood high cut reduction viscosity,whole blood low cut reduction viscosity,plasma viscosity,RBC aggregation index,red blood cell rigidity index and the blood cell rigidity index were significantly lower in the two groups.The levels of RBC deformation index,Fg and HCT were significantly lower than those in the control group(P<0.05).The incidence of complications in the observation group was 5.00%(2/40),significantly lower than that(20.00%,8/40)of the control group.The incidence of adverse reactions in the observation group was 7.50%(3/40),significantly lower than that of the control group(25.00%,10/40)(P<0.05).Conclusion Shenmai Injection has a significant effect on coronary heart disease and can obviously alleviate the symptoms of patients,improve the heart function and the blood flow function of patients,and it is safe and reliable,and can be widely used in clinic.
