摘要
目的基于中医传承辅助平台(V2.5)系统,挖掘、分析穴位贴敷治疗慢性阻塞性肺疾病的现代临床文献,探究其选穴配伍规律,为穴位贴敷治疗慢阻肺提供新思路、新方案。方法检索中国学术期刊全文数据库(CNKI)建库至2018年10月所有公开发表的穴位贴敷治疗慢阻肺相关的临床研究文献,经过筛选、提取,得出其有效的穴位处方并录入中医传承辅助平台,借助软件中一般统计法、文本挖掘、关联规则分析、复杂系统熵聚类等数据挖掘方法,对其穴位处方的核心选穴、组方规律进行深度分析。结果共筛选出有效的慢阻肺穴位贴敷处方259张,其中包含穴位70个,穴位出现频次≥10次的共16个;设置支持度为20%、置信度为0.65,运用关联规则分析得出核心穴位组合16个,包含穴位9个,穴位间关联规则18条;设置相关度为8、惩罚度为2,根据复杂系统熵聚类衍化出内在核心穴位组合18个,新方组合9首。结论穴位贴敷治疗慢性阻塞性肺疾病的高频腧穴是肺俞、定喘、天突、大椎、肾俞、脾俞和膻中等,肺俞最为常用;循经选穴以足太阳膀胱经、任督二脉、经外奇穴和足阳明胃经为主,同时主以近部选穴,辅以辨证选穴、远部选穴及对症选穴,既彰显外治特色,又不废内治规律;扶正与祛邪兼顾,重视选用任督经穴扶正固本。
Objective:to excavate and analyze the modern clinical literature of acupoint application for COPD,based on V2.5 system of traditional Chinese Medicine Inheritance assistant platform,and to explore the law of acupoint selection and compatibility,so as to provide a new idea and scheme for acupoint application in the treatment of COPD.Methods:All published clinical research literatures related to acupoint application for COPD from the establishment of CNKI until October 2018 were Retrieved.Through screening and extraction,the effective acupoint prescriptions were obtained and input into the assistant platform of traditional Chinese medicine inheritance.With the help of data mining methods such as general statistical method,text mining,association rule analysis and complex system entropy clustering,the core acupoint selection and prescription rule of acupoint prescriptions were deeply analyzed.Results:A total of 259 effective acupoint application prescriptions for COPD were screened out,including 70 acupoints and 16 acupoints with frequency of occurrence more than 10 times.The support degree was 20% and the confidence degree was 0.65.According to association rule analysis,16 core acupoint combinations were obtained,including 9 acupoints and 18 association rules between acupoints.Setting correlation degree to 8 and punishment degree to 2,18 inner core acupoint combinations and 9 new combinations were derived from complex system entropy clustering.Conclusion:The high frequency acupoints of acupoint application for chronic obstructive pulmonary disease are Feishu,Dingchuan,Tiantuo,Dazhui,Shenshu,Pishu and Shanzhong,and Feishu are most commonly used.The main points selected along the meridians are the Foot-Taiyang Bladder Meridian,Ren-Du Meridian,Waiqi Meridian and Foot-Yangming Stomach Meridian.At the same time,the main points are selected in the proximal part,supplemented by syndrome differentiation,distal selection and symptomatic selection,which not only highlights the characteristics of external treatment,but also does not abolish the rule of internal treatment.Strengthening the body resistance and eliminating evil are paid attention to,and the selection of Ren-Du Meridian is emphasized.
