摘要
目的:数据挖掘法分析、总结中医药物治疗慢性阻塞性肺疾病的用药规律,并对中药用药规律相关中医机制进行探讨分析。方法:根据慢性阻塞性肺疾病的诊断要点或疾病特征,筛选《中医方剂大辞典》中关于"咳嗽""咳痰""气短""喘息"及"肺胀"治疗慢性阻塞性肺疾病相关的口服方剂,运用"中医传承辅助系统(V2.50)",进行用药规律分析。结果:共纳入方剂92首,涉及中药189味,挖掘出高频药物人参、桑白皮、桔梗、麦冬、紫菀、杏仁、款冬花、川贝母、大枣、半夏、麻黄等扶助肺气肺阴、止咳化痰类中药,辨证治疗以"化痰祛邪、益气扶正"为基本大法。结论:总结用药规律,印证了"正气亏虚""痰实阻肺"等本虚标实之病机贯穿于疾病演变过程中;"益气化痰"治则指导慢性阻塞性肺疾病急性加重及稳定期的治疗始末。最终得出,依据慢性阻塞性肺疾病"本虚标实"的复合证型特点,在用药时可根据具体病情给予"益气""化痰"治疗的组合,进行整体调护,促进疾病尽早向愈。
Objective:To explore and analyze the mechanism of traditional Chinese medicine in the treatment of chronic obstructive pulmonary disease.Methods:According to the diagnostic characteristics of chronic obstructive pulmonary disease,the oral formulations of"cough","cough phlegm","shortness of breath","wheezing"and"lung distension"in the dictionary of traditional Chinese medicine prescription for the treatment of chronic obstructive pulmonary disease were selected,and the medication rules were analyzed by using"TCM inheritance auxiliary system(V2.50)".Results:A total of 92 prescriptions were included,involving 189 flavors of traditional Chinese medicine,and the basic idea of"removing phlegm,removing evil spirits and improving Qi"was summarized for the treatment of syndrome differentiation.Conclusion:By summarizing the law of medication,it is proved pathogenesis is deficiency of vital Qi and obstructing lung by phlegm,etc.The principle of"invigorating Qi and removing phlegm"is to guide the treatment of acute exacerbation and stabilization of chronic obstructive pulmonary disease.Finally,according to the complex syndrome types of chronic obstructive pulmonary disease(COPD),the combination of"beneficial Qi"and"eliminating phlegm"can be given according to the specific conditions when using drugs to carry out overall nursing and promote the early recovery of the disease.
