摘要
目的探讨丁苯酞联合依达拉奉对老年急性脑梗死患者抗氧化能力、炎性反应及神经功能恢复的影响。方法将150例急性脑梗死患者随机分为对照A组、对照B组、观察组各50例,均给予常规治疗,对照A组、对照B组、观察组分别加用血栓通、依达拉奉、依达拉奉联合丁苯酞治疗。比较3组治疗后总有效率,治疗前及治疗后血清神经元特异性烯醇化酶(NSE)、单核细胞趋化和激活因子(MCAF)、S100-β蛋白、丙二醛(MDA)、超氧化物歧化酶(SOD)、总抗氧化能力(T-AOC)、白细胞介素(IL)-8、IL-10、C反应蛋白(CRP)水平和梗死灶体积、美国国立卫生研究院脑卒中量表(NIHSS)、改良Rankin量表(mRS)评分。结果 (1)对照A组、对照B组、观察组的总有效率依次升高(均P<0.05)。(2)随时间延长,各组NSE、MCAF、S100-β蛋白水平均有下降的趋势,SOD、T-AOC水平有升高的趋势(均P<0.05);治疗后7 d及14 d,对照A组、对照B组、观察组NSE、MCAF、S100-β蛋白、MDA水平依次降低,而SOD、T-AOC蛋白水平依次升高(均P<0.05)。(3)各组IL-8、IL-10及CRP水平均有随时间延长而降低的趋势(均P<0.05);观察组治疗后7 d及14 d的IL-8、治疗后7 d的CRP水平均低于其他两组(均P<0.05),治疗后14 d对照A组、对照B组、观察组IL-10及CRP水平依次降低(均P<0.05)。(4)治疗后,观察组NIHSS及mRS评分均低于治疗前水平及其他两组(均P<0.05),梗死灶体积低于其他两组(均P<0.05)。结论丁苯酞联合依达拉奉治疗老年急性脑梗死具有较好的临床疗效,可进一步提高患者的抗氧化能力、降低炎性反应水平,促进神经功能的恢复。
Objective To investigate the effects of butylphthalide combined with edaravone on antioxidant effect,inflammatory response ,and recovery of nerve function in the elderly patients with acute cerebral infarction. Methods A total of 150 elderly patients with acute cerebral infarction were randomly divided into control group A,control group B and observation group,with 50 cases in each group.The three groups received routine therapy.Besides,the control group A,control group B and observation group were administered Xueshuantong,edaravone,and edaravone combined with butylphthalide respectively.The related indices were compared among the three groups,including the total effective rate after treatment,the levels of serum neuron-specific enolase(NSE),monocyte chemotactic and activating factor(MCAF),S100-β protein,malondialdehyde(MDA),superoxide dismutase(SOD),total antioxidant capacity(T-AOC), interleukin (IL)-8,IL-10 and C-reactive protein(CRP),infarct volume,the National Institutes of Health Stroke Scale(NIHSS) and modified Rankin Scale(mRS) scores before and after treatment. Results The total effective rate increased in the order of the control group A,control group B and observation group(all P 〈0.05) (2) In each group,the levels of NSE,MCAF and S100-β protein decreased and the levels of SOD and T-AOC increased by the time(all P 〈0.05).At 7 and 14 days after treatment,the levels of NSE,MCAF,S100-β protein and MDA decreased and the levels of SOD and T-AOC protein increased in the order of the control group A,control group B and observation group(all P 〈0.05).(3) In each group,the levels of IL-8,IL-10 and CRP decreased by the time(all P 〈0.05).The IL-8 levels at 7 and 14 days after treatment and the CRP level at 14 days after treatment were lower in the observation group compared to the two other groups(all P 〈0.05).At 14 days after treatment,the IL-10 and CRP levels decreased in the order of the control group A,control group B and observation group(all P 〈0.05).(4) After treatment,the NIHSS and mRS scores of the observation group were lower than the scores before treatment and the scores of the other two groups(all P 〈0.05),and the infarct volume in the observation group was lower than that in the other two groups(all P 〈0.05). Conclusion Butylphthalide combined with edaravone can obtain a good clinical efficacy for the elderly patients with acute cerebral infarction,can improve the patient′s antioxidant effect further,reduce the inflammatory response and promote the recovery of nerve function.
