摘要
目的:探究不同剂量丁苯酞对急性脑梗死(CI)患者脂肪因子、氧化应激及炎症反应的影响。方法:研究对象为2015年4月—2016年7月我院收治的114例CI患者,按随机数字表法分为三组,各38例。三组均给予基础治疗,在基础治疗上,低剂量组给予丁苯酞氯化钠注射液(静脉滴注,100 mL,bid);高剂量组给予丁苯酞氯化钠注射液(静脉滴注,200 mL,bid);三组均治疗2周。比较三组治疗前后血清超敏C-反应蛋白(hs-CRP)水平、超氧化物歧化醇(SOD)、丙二醇(MDA)、基质金属蛋白酶(MMP)-9、肿瘤坏死因子(TNF)-α、白细胞介素(IL)-8、脂联素和趋化素水平。结果:治疗后,三组TNF-α、IL-8、hsCRP、MMP-9、MDA、趋化素水平均低于治疗前,且高剂量组TNF-α、MMP-9、MDA、趋化素水平低于低剂量组与对照组,SOD、脂联素水平均高于治疗前,且高剂量组高于低剂量组与对照组,差异有统计学意义(P<0.05)。结论:高剂量丁苯酞治疗CI的效果较好,可减轻患者的氧化应激反应,抑制炎症反应,改善脂肪因子异常。
Objective:To investigate the effect of different doses of butylphthalide on adipokines,oxidative stress and inflammatory response in patients with acute cerebral infarction(CI).Methods:114 patients with CI admitted to our hospital from April 2015 to July 2016 were used as subjects investigated,and they were randomly divided into three groups,38 cases each.The patients in the three groups were given basic treatment.On the basis of the basic treatment,the patients in the low-dose group were given butylphthalide sodium chloride injection(intravenous drip,100 mL,bid),and the patients in the high-dose group were given butylphthalide sodium chloride injection(intravenous drip,200 mL,bid).All the patients in the three groups were treated for 2 weeks.The levels of serum high-sensitivity C-reactive protein(hs-CRP),superoxide dismutase(SOD),propylene glycol(MDA),matrix metalloproteinase(MMP)-9,tumor necrosis factor(TNF)-α,interleukin(IL)-8,adiponectin and chemokine before and after treatment in the three groups were compared.Results:The levels of TNF-α,IL-8,hs-CRP,MMP-9,MDA and chemokine after treatment were lower than those before treatment in the three groups,and the levels of TNF-α,MMP-9,MDA and chemokine in the high-dose group were lower than those in the low-dose group and the control group,while the levels of SOD and adiponectin after treatment were higher than those before treatment,and the levels in the high-dose group were higher than those in the low-dose group and the control group,with statistically significant differences(P<0.05).Conclusion:High-dose butylphthalide has a better therapeutic effect on CI,which can alleviate oxidative stress response,inhibit inflammatory response and improve the abnormality of adipokines.
