摘要
目的:通过临床流行病学调查了解冠心病不稳定性心绞痛患者的中医证候要素分布规律。方法:对411例住院患者经冠脉造影确诊为冠心病的冠心病不稳定性心绞痛患者进行病例调查,运用描述性统计分析法和χ2检验分析法对中医证候要素、常见症状、舌脉象与冠脉造影结果的相关性进行了分析。结果:冠心病不稳定性心绞痛患者中医证候要素血瘀占比例最大,占72.7%;其次为气虚和痰浊,分别占48.4%、22.9%。分析显示,冠状动脉不同病变支数间舌象、脉象,均以舌暗红或淡暗、苔白或薄白、脉沉、细、弦为主。结论:以血瘀为核心,以气虚和痰浊为重要致病要素为冠心病的病机所在,同时需要注意伴随症状的影响,辨证施治,且冠脉病变支数与舌象、脉象之间的相关性分析差异有统计学意义。
Objective: To study the distribution of syndrome factors of TCM in patients with unstable angina pectoris of coronary artery disease by clinical epidemiological investigation. Methods: Investigate 411 cases of hospitalized patients who were diagnosed as unstable angina pectoris of coronary artery disease by coronary angiography to carry out the correlation between the results of coronary angiography with syndrome factors of TCM, common symptoms and tongue manifestation of patients by descriptive statistical analysis and χ2 test correlation analysis. Results: Unstable angina pectoris of coronary artery disease with TCM syndrome factors of blood stasis accounted for the largest proportion(72.7%), and the followed was qi deficiency and phlegm turbidity(48.4% and 22.9%, respectively). Additionally, analysis results showed that the expression of tongue manifestations take the dark red or pale dark tongue and moss white or white fur mainly, also pulse manifestations take the deep, thread and stringy mainly. Conclusion: The blood stasis caused by the qi deficiency and phlegm played impor tant roles in the pathogenesis of unstable angina pectoris of coronary artery disease. It requires attention for the influences of accompanied symptoms in treatment based on syndrome differentiation. In addition, there were close correlation between the coronary artery lesion counts and the tongue or pulse manifestations.
