摘要
目的:探讨预防应用聚乙二醇化重组人粒细胞刺激因子(PEG-rhG-CSF)对降低宫颈癌同步放化疗期间中性粒细胞(ANC)减少的临床价值。方法:选取河南省肿瘤医院2017年10月至2018年5月收入的行根治性同步放化疗的宫颈癌患者60例,随机分为2组:研究组(30例)于同步放化疗首次出现ANC减少时给予PEG-rhG-CSF 6mg皮下注射+重组人粒细胞集落刺激因子(rhG-CSF)150μg皮下注射;对照组(30例)于同步放化疗开始后首次出现ANC减少时给予rhG-CSF 150μg皮下注射。分析上述各组患者各个时间的ANC、ANC减少导致放化疗延迟发生率及不良反应缓解时间。结果:研究组患者给药后各个时间的ANC明显高于对照组,差异有统计学意义(P<0.05)。研究组患者在给药48 h后ANC可基本恢复正常。两组患者给药后因ANC减少导致放化疗延迟的发生率分别为23.3%(7/30)、63.3%(19/30),差异有统计学意义(χ~2=9.774,P<0.05);两组患者给药后因药物不良反应差异无统计学意义(P>0.05),对于给药后出现发热、骨痛症状的缓解时间,研究组患者较对照组患者明显缩短(P<0.05)。结论:预防应用PEG-rhG-CSF可降低ANC减少发生,对减少宫颈癌患者同步放化疗中因骨髓抑制所致的治疗间断、提高放化疗疗效有重要临床价值。
Objective:To investigate the clinical value of pegylated recombinant human granulocyte colonystimulating factor(PEG-rh G-CSF)in reducing neutrophils deficiency of cervical cancer with concurrent chemoradiotherapy.Methods:A total of 60 patients with cervical cancer who were treated by concurrent chemoradiotherapy from October 2017 to May 2018 were enrolled and randomely divided into two groups:30 cases in control group and 30 cases in observation group.The control was received subcutaneous injections of 150μg rh G-CSF when there was a neutrophils deficiency based on concurrent chemoradiotherapy,while the observation underwent subcutaneous injection of 150μg rh G-CSF plus 6 mg PEG-rh G-CSF.The absolute neutrophil count(ANC)at different time points,the rate of radiotherapy and chemotherapy delay induced by neutrophils deficiency,and the period of adverse reactions relief were compared between the two groups by variance analysis,χ2 test and t test.Results:The ANC in observation group at different time points were significantly higher than in control group(P<0.05).The conditions of neutropenia were improved after 48 h and 120 h in observation group as compared with that in control group.The rate of radiotherapy and chemotherapy delay induced by neutrophils deficiency in observation group and control group were 23.3%(7/30)and 63.3%(19/30),respectively,with significant difference(χ~2=9.774,P<0.05).There was no statistical significance in rate of drugs adverse reactions in both groups(P>0.05).Compared with that in control group,the period of the symptom relief such as fever and skeletal muscle pain in observation group was shortened obviously(P<0.05).Conclusion:The PEG-rh G-CSF can reduce the incidence of neutrophils deficiency.It’s valuable in the reduction of treatment interruption due to myelosuppression in concurrent chemoradiotherapy for cervical cancer patients,thus improving the chemoradiotherapy curative effect.
