摘要
目的分析周口市新型冠状病毒肺炎(COVID-19)聚集性疫情流行病学特征,为下一步疫情防控提供科学依据。方法采用描述性分析方法对2020年1月23日-6月30日周口市新型冠状病毒肺炎聚集性疫情进行流行病学分析。结果周口市共发生聚集性疫情13起,涉及42例确诊病例和1例无症状感染者,占总确诊病例数的55.84%,11起(84.62%)为家庭聚集性。聚集规模为2~5例/起。15~59岁组发病较多,占86.05%;男女比例2.12∶1;潜伏期中位数5.0 d。首发病例发病前14 d内有武汉旅居史的9起(69.23%);聚集性疫情代际传播中仅有1起出现第三代病例;治愈病例中出现3例(6.98%)核酸复阳病例;1名无症状感染者核酸阳性持续64 d。结论周口市聚集性疫情以输入为主,聚集规模小,代际传播链条短,家庭聚集是聚集性疫情的主要类型;下一步应坚持"外防输入,内防扩散"策略,提高医疗机构及早发现病例能力,加强病例治愈出院后隔离管理,对密切接触者实行集中隔离管理,防止家庭内部传播。
Objective To analyze epidemiological characteristics of COVID-19 clustering cases in Zhoukou for providing evidences for the next step of epidemic prevention and control.Methods Descriptive analysis was used to analyze COVID-19 cluster in Zhoukou.Results As of June 30,2020,there had been a total of 13 clusters involving 42 confirmed cases and 1 asymptomatic infection in Zhoukou city,accounting for 55.84 percent of the total confirmed cases,11 of which were family clusters.The cluster size was 2-5 cases per cluster.The 15-59 years old group had a large number of cases,accounting for86.05%of the total cluster cases.The male to female ratio was 2.12∶1;The median incubation period was 5.0 days.9 cases(69.23%)had a residence history in Wuhan within 14 days before the onset of the first case.In the intergenerational transmission of the clustered epidemic,there was only one third-generation case.There were 3 cases(6.98%)having recurrence of positive nucleic acid test among the cured cases.One asymptomatic infected case remained nucleic acid test positive for 64 days.Conclusion The cluster epidemic in Zhoukou was mainly imported,with small cluster size and short intergenerational transmission chain.Family clustering was the main cause of the cluster epidemic.The next step should be adhered to the strategy of"preventing imported cases and local spread",improve the ability of medical institutions to detect cases early,strengthen the isolation management of cured and discharged patients,and implement centralized isolation management for close contacts to prevent intra-family transmission.
