摘要
目的探讨花生四烯酸(AA)、二磷酸腺苷(ADP)诱导的血小板聚集功能与RSA的相关性。方法纳入了2023年10月至2024年12月期间在北京妇产医院被诊断为RSA的未孕女性217例作为研究组。并选取同期具有正常分娩史的健康未孕女性200例作为对照组。比较两组AA、ADP诱导的血小板聚集功能的水平;通过ROC曲线分析AA、ADP诱导的血小板聚集功能对RSA的预测价值;通过多因素Logistic回归分析发生RSA的影响因素。结果复发性流产组AA、ADP诱导的血小板聚集功能均显著高于健康对照组(P<0.05);ROC曲线分析表明,AA、ADP诱导的血小板聚集功能对RSA的独立预测效能AUC分别为0.95(95%CI:0.92~0.97)、0.97(95%CI:0.95~0.99);多因素Logistic回归分析的结果显示,AA、ADP诱导的血小板聚集高水平是发生RSA的独立危险因素(P<0.05)。结论AA、ADP诱导的血小板聚集功能在RSA患者中显著升高,两个指标的检测对于预测RSA具有一定的诊断意义。
Objective The aim of this study is to investigate the correlation between platelet aggregation function induced by arachidonic acid(AA)and adenosine diphosphate(ADP)and RSA.Methods This study included 217 non-pregnant women diagnosed with RSA at Beijing Obstetrics and Gynecology Hospital from October 2023 to December 2024 as the research group.In addition,200 healthy non-pregnant women with a normal history of childbirth during the same period were selected as the control group.We compared levels of platelet aggregation function induced by AA and ADP between two groups.Specifically,we evaluated the predictive value of AA and ADP induced platelet aggregation function for RSA through ROC curve analysis;We identified influencing factors of RSA through multiple logistic regression analysis.Results The platelet aggregation function induced by AA and ADP in the recurrent miscarriage group was significantly higher than that in the healthy control group(P<0.05);ROC curve showed that the independent prediction AUC of AA and ADP induced platelet aggregation function for RSA were 0.95(95%CI:0.92-0.97)and 0.97(95%CI:0.95-0.99),respectively;The results of multiple logistic regression analysis showed that high levels of platelet aggregation induced by AA and ADP were independent risk factors for the occurrence of RSA(P<0.05).Conclusion The platelet aggregation function induced by AA and ADP is significantly elevated in RSA patients,and the detection of these two indicators has certain diagnostic significance for predicting RSA.
