A hepatitis B sero-epidemiological survey of 1-79-year-olds in Henan province in 2023

摘要 目的 分析河南省1~79岁人群乙型肝炎(乙肝)血清流行率。方法 2023年8-11月在河南省18个省辖市选取29个村/社区的1~79岁人群开展问卷调查并采集血清标本,采用酶联免疫吸附试验检测乙肝表面抗原(Hepatitis B surface antigen,HBsAg)、乙肝表面抗体(Hepatitis B surface antibody,HBsAb)和乙肝核心抗体(Hepatitis B core antibody,HBcAb),分析血清流行率。结果 共纳入调查对象11 883例,HBsAg、HBsAb、HBcAb阳性率分别为1.24%、69.49%、25.40%,标化阳性率分别为1.98%、62.62%、32.28%。各年龄组HBsAg、HBsAb、HBcAb阳性率分别在0.23%~3.75%、46.92%~88.07%、16.30%~59.05%之间(χ^(2)=149.73,P<0.001;χ^(2)=1 146.45,P<0.001;χ^(2)=732.02,P<0.001);其中1~19岁、20~79岁各年龄组HBsAg阳性率分别在0.23%~0.56%、1.20%~3.75%之间,15~19岁、55~59岁HBsAb阳性率最低,分别为46.92%、53.36%。结论 2023年河南省1~79岁人群乙肝感染总体处于低流行水平,<19岁人群感染率低,易感高峰年龄为15~19岁和55~59岁。需继续保持儿童乙肝疫苗高水平接种率,探讨高易感人群乙肝疫苗加强或补充免疫的必要性。 Objective To analyze hepatitis B seroprevalence among people aged 1-79 years in Henan province.Methods We selected 1-79-year-olds from 29 villages/communities in 18 prefectures of Henan for a questionnaire-based survey,and obtained serum samples between August and November 2023.We tested for hepatitis B surface antigen(HBsAg),surface antibody(HBsAb),and core antibody(HBcAb)using enzyme-linked immunosorbent assay to determine seroprevalences.Results A total of 11883 subjects were included,in which HBsAg,HBsAb,and HBcAb positivity rates were 1.24%,69.49%,and 25.40%(adjusted:1.98%,62.62%,and 32.28%),respectively.HBsAg,HBsAb,and HBcAb positivity by age group ranged from 0.23%to 3.75%,46.92%to 88.07%,and 16.30%to 59.05%(χ^(2)=149.73,P<0.001;χ^(2)=1146.45,P<0.001;χ^(2)=732.02,P<0.001).HBsAg positivity ranges among 1-19-and 20-79-year-olds were 0.23%to 0.56%and 1.20%to 3.75%.HBsAb positivity rates for 15-19-and 55-59-year-olds s were the lowest at 46.92%and 53.36%.Conclusions The prevalence of hepatitis B infection generally remained low among people aged 1-79 years in Henan in 2023,particularly among<19-year-olds.Susceptibility peaked in 15-19-and 55-59-year-olds.It is necessary to continuously maintain high coverage with childhood hepatitis B vaccination and explore the necessity of booster doses or supplementary hepatitis B vaccination for highly susceptible populations.
