摘要
目的分析陕西省铜川市1~59岁健康人群乙型肝炎(乙肝)表面抗体(Hepatitis B surface antibody,HBsAb)水平。方法采用多阶段分层随机抽样方法,于2023年5-8月在铜川市4个区县8个村/社区采集1~59岁健康人群血清标本,采用电化学发光免疫试验检测HBsAb,分析阳性率和几何平均浓度(Geometric mean concentration,GMC)。结果调查对象HBs Ab总阳性率为54.93%(1058/1926),标化阳性率为54.57%;总GMC为42.36 m IU/m L。1~4岁、5~14岁、15~29岁、30~59岁年龄组HBsAb阳性率分别为87.10%、56.52%、48.57%、52.97%。0剂次、1剂次、2剂次、3剂次、≥4剂次乙肝疫苗(Hepatitis B vaccine,HepB)免疫史者阳性率分别为40.57%、49.07%、54.05%、61.22%、86.05%。在有HepB免疫史调查对象中,接种后<1年、1~4年、5~9年、10~14年、15~19年、≥20年HBsAb阳性率分别为78.42%、82.33%、74.26%、58.78%、39.25%、29.63%。结论2023年铜川市1~59岁健康人群HBsAb总体处于高水平,存在一定既往感染和易感者;5~14岁年龄组或接种HepB后5~9年抗体水平显著下降,建议探讨该人群乙肝标志物筛查、HepB加强或补充免疫等措施。
ObjectiveTo analyze hepatitis B surface antibody(HBsAb)levels among healthy 1-59-yearolds in Tongchuan city,Shaanxi province.MethodsWe used a multi-stage stratified random sampling method to select healthy individuals aged 1-59 years from 8 villages/communities in 4 districts/counties of Tongchuan between May and August 2023 and collect serum samples.We tested sera for HBsAb using electrochemiluminescence immunoassay to determine positivity rates and geometric mean concentrations(GMC).ResultsOverall HBsAb positivity was 54.93%(1058/1926,adjusted rate:54.57%)with a GMC of 42.36 m IU/m L.Positivity rates among 1-4-,5-14-,15-29-,and 30-59-year-olds were 87.10%,56.52%,48.57%,and 52.97%,respectively.Positivity rates among subjects who had received 0,1,2,3,and≥4 doses of hepatitis B vaccine(HepB)were 40.57%,49.07%,54.05%,61.22%,and 86.05%.Among subjects who had received HepB,positivity rates at<1,1-4,5-9,10-14,15-19,and≥20 years after vaccination were 78.42%,82.33%,74.26%,58.78%,39.25%,and 29.63%.ConclusionsHBsAb levels were generally high among healthy individuals aged 1-59 years in Tongchuan in 2023.Antibody levels decreased significantly among 5-14-year-olds or 5-9 years after HepB vaccination.We recommend exploring measures such as screening for hepatitis B markers and use of booster doses or supplementary HepB vaccination for this population.
