摘要
目的 探讨经皮雌二醇凝胶和口服戊酸雌二醇片对高龄女性冻融胚胎移植(FET)临床结局的影响。方法回顾性分析2021年1月至2022年7月于江西省妇幼保健院辅助生殖中心进行FET治疗的391例高龄(≥38岁)女性患者的临床资料,将在内膜准备中经皮外用雌二醇凝胶的患者归为经皮组(n=192),口服戊酸雌二醇片的患者归为口服组(n=199),比较2组的临床特征。结果 2组患者年龄、体重指数、窦卵泡计数、抗缪勒管激素比较差异无统计学意义(P>0.05)、且其治疗情况(转化日内膜厚度、凝血功能、肝功能)及临床结局(HCG阳性率、生化流产率、临床妊娠率、多胎率、种植率、流产率、活产率)比较,其差异亦无统计学意义(P>0.05)。结论 在高龄女性FET助孕过程中经皮雌二醇凝胶与口服戊酸雌二醇片给药具有相似的临床结局和较好的安全性。
Objective To investigate the effects of transdermal estradiol gel and oral estradiol tablets on clinical outcomes of frozen embryo transfer(FET)in older women.Methods The clinical data of 391 older(≥38 years old)female patients who underwent FET at the Center for Assisted Reproduction of Jiangxi Maternal and Child Health Hospital from January 2021 to July 2022 were retrospectively analyzed;the patients who used estradiol gel percutaneously and externally in endothelial preparations were classified as the estradiol gel group(n=192),and the patients who took estradiol valerate tablets orally were classified as the oral estradiol group(n=199);the clinical characteristics of the 2 groups were compared.Results There were no statistically signifi-cant differences in age,body mass index,sinus follicle count,and anti-mullerian hormone between the 2 groups(P>0.05),and there were no statistically significant differences in treatment(endo-thelial thickness on the day of conversion,coagulation function,and hepatic function)and clinical outcomes(HCG positive rate,biochemical miscarriage rate,clinical pregnancy rate,multiple pregnancy rate,implantation rate,miscarriage rate,and live-birth rate)(P>0.05).Conclusion Trans-dermal estradiol gel and oral estradiol tablet might have similar clinical outcomes during FET pregnancy assistance in older women,both with a desirable safety profile.
