摘要
目的探讨补肾化浊活血方联合盐酸美金刚对血管性痴呆患者脑血流动力学及认知功能的影响。方法选取2019年6月至2022年4月医院确诊为血管性痴呆的62例患者作为研究对象。将上述患者录入计算机系统,随机编号排序,将序号奇数位纳入对照组(31例)接受盐酸美金刚治疗,将序号偶数位纳入观察组(31例)接受补肾化浊活血方联合盐酸美金刚治疗,两组患者均治疗6个疗程,每个疗程4周。比较两组患者的治疗有效率以及治疗前后的脑血流动力学指标[大脑前动脉血流平均流速(V_(前))、大脑中动脉血流平均流速(V_(中))、大脑后动脉血流平均流速(V_(后))]、认知功能[蒙特利尔认知评估量表(MOCA)、认知功能障碍评分(MMSE)]、中医证候[血管性痴呆辨证量表(SDSVD)]、血清指标[脑源性神经营养因子(BDNF)、神经元特异性烯醇化酶(NSE)、神经生长因子(NGF)]、不良反应情况。结果治疗后,观察组总有效率高于对照组(P<0.05);治疗后观察组患者V_(前)指标、V_(中)指标、V_(后)指标、MOCA评分、MMSE评分、BDNF指标、NGF指标均高于对照组(P<0.05);观察组患者的SDSVD评分、NSE指标均低于对照组(P<0.05)。两组患者的不良反应率比较差异无统计学意义(P>0.05)。结论补肾化浊活血方联合盐酸美金刚对血管性痴呆患者效用良好,可有效改善患者脑血流动力学、临床症状、神经功能以及认知功能且安全性良好。
Objective To explore the effects of Bushen Huazhuo Huoxue Decoction combined with memantine hydrochloride on cerebral hemodynamics and cognitive function in patients with vascular dementia.Methods A total of 62 patients with vascular dementia confirmed in the hospital were enrolled as the research objects from June 2019 to April 2022.Patients were input into the computer system,random number sorting,the serial number odd included in the control group(31 cases)for memantine hydrochloride treatment,the serial number even included in the observation group(31 cases)for Bushen Huazhuo Huoxue Decoction combined with memantine hydrochloride treatment.All were treated for 6 courses of treatment(4 weeks/course).The response rate of treatment,cerebral hemodynamics indexes[mean flow velocity of anterior cerebral artery(V_(anterior)),mean flow velocity of middle cerebral artery(V_(middle)),mean flow velocity of posterior cerebral artery(V_(posterior))],cognitive function[scores of Montreal cognitive assessment scale(MOCA)and mini-mental state examination(MMSE)],TCM symptoms[scale for the differentiation of syndromes of vascular dementia(SDSVD)]and serum indexes[brain-derived neurotrophic factor(BDNF),neuron-specific enolase(NSE),nerve growth factor(NGF)]before and after treatment,and adverse reactions in the two groups were compared.Results After treatment,total response rate of observation group was higher than that of control group(P<0.05).After treatment,V_(anterior),V_(middle),V_(posterior),MOCA score,MMSE score,BDNF and NGF were higher in observation group than control group(P<0.05),while SDSVD score and NSE were lower in observation group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The application effect of Bushen Huazhuo Huoxue decoction combined with memantine hydrochloride is good in patients with vascular dementia,which can effectively improve cerebral hemodynamics,clinical symptoms,nerve function and cognitive function,with good safety.
