摘要
目的通过知识图谱技术分析胡镜清研究员治疗痴呆的临证思路和特色,为痴呆中医临床诊疗及中医医案研究方法提供参考。方法以胡镜清研究员治疗痴呆的门诊医案为研究对象,进行知识图谱的模式层和数据层设计,运用Cypher语言在Neo4j图数据库中设计一种符合中医临床思维模式的知识图谱框架。结果构建了包含5类、139个实体,4类、277条关系的“分期-证候-临床表现-方剂-中药”痴呆诊疗知识图谱,并通过查询功能实现对所需痴呆证治知识的快速检索。发现胡镜清研究员论治痴呆首辨“平台期”与“波动期”,平台期侧重从“虚”论治,善用六味地黄丸、参苓白术散、定志丸、六君子汤等;波动期侧重从“火热”“痰瘀化热”论治,代表方剂为黄连温胆汤、桃红四物汤、自拟清热方;同时亦不忘使用五苓散从“温阳利水”角度辨治痴呆。结论利用知识图谱技术能够有效分析和可视化展示胡镜清研究员论治痴呆的临证思路和特色,为痴呆中医临床诊疗提供参考,进一步为中医医案的合理有效利用提供方法。
Objective To analyze the clinical thinking of researcher Hu Jingqing(Hereinafter referred to as Dr.Hu)in the treatment of dementia by using knowledge graph technique,and to provide guidance for clinical treatment.Methods Taking the outpatient patient of Dr.Hu′s treatment of dementia as the research object,the pattern layer and the data layer of the knowledge graph were designed,and a knowledge graph framework was designed to simulate the thinking pattern of human brain in the Neo4j graph database using CYPHER language.Results The knowledge map of Dr.Hu′s medical cases for dementia was constructed,including 139 entities in 5 categories,277 relationships in 4 categories,which could be quickly retrieved by entities and relationships.Conclusion Master Hu divided dementia into plateau stage and fluctuating stage.The plateau stage was treated from"deficiency",and Liuwei Dihuang pill,Shenling Baizhu powder,Dingzhi pill and Liujunzi decoction were well used.In the fluctuating period,the treatment was based on"mutual association of phlegm and blood stasis",and"hot",and the representative prescriptions were Huanglian Wendan Decoction,Taohong Siwu Decoction and Self-drafted heat clearing prescription.Use"Wuling San"to distinguish and treat dementia from the Angle of"warming Yang and benefiting water".
