摘要
糖尿病会引发一系列并发症,像糖尿病肾病、糖尿病足、糖尿病皮肤病、糖尿病视网膜病变等,而糖尿病周围神经病变是由糖尿病发展而来的慢性并发症的一种。患者症状多见对称性疼痛和感觉异常,下肢症状较上肢多见。在治疗糖尿病的过程中,除了控制好血糖以外,对其慢性并发症要给予及时干预,以避免发展为更为严重的情况。中医学认为糖尿病周围神经病变发生与消渴日久、耗伤气阴、营卫不调、气血运行不畅、络脉瘀阻相关。历代医家对糖尿病周围神经病变有不同理解,主要症状为“麻、痛、冷、疲”等,其病理机制多为“寒、湿、虚、瘀”。糖尿病周围神经病变属本虚标实之证,治疗之法亦应取或补、或攻、或攻补兼施应随证治之。木丹颗粒具有活血化瘀、通脉止疼的功效,在治疗糖尿病周围神经病变方面应用广泛。文章从临床研究、实验研究、循证医学研究和现代药理学研究四个纬度对相关文献进行梳理,综述木丹颗粒治疗糖尿病周围神经病变的研究进展。
Diabetes can lead to a series of complications,such as diabetic nephropathy,diabetic foot,diabetic skin disease,diabetic retinopathy,and diabetic peripheral neuropathy is a chronic complication developed from diabetes.Symmetrical pain and paresthesia were more common in patients,and lower limb symptoms were more common than upper limb symptoms.In the process of treating diabetes,in addition to controlling blood glucose,timely intervention should be given to its chronic complications to avoid the development of more serious conditions.TCM believes that the occurrence of DPN is related to thirst quenching for a long time,consuming Qi(气)and Yin(阴),irregular guarding,poor flow of Qi and blood,and obstruction of veins.Generations of doctors have different understandings of DPN,the main symptoms are numbness,pain,cold,fatigue,etc.,and its pathological mechanism is mostly cold,dampness,deficiency,stasis.DPN is the evidence of this false standard,and the method of treatment should also be taken or supplemented,or attacked,or attacked and supplemented,and should be treated with symptom.The Mudan granules(木丹颗粒)have the effect of promoting blood circulation to remove blood stasis,dredging and relieving pain,and were widely used in the treatment of diabetic peripheral neuropathy.This article reviews the relevant literature from four latitudes of clinical research,experimental research,evidence-based medicine research and modern pharmacology research,and summarizes the research progress of the Mudan granules in the treatment of diabetic peripheral neuropathy.
