摘要
目的比较保守治疗后不同时机行腹腔镜阑尾切除术治疗化脓性阑尾炎的临床效果。方法选择2014年8月至2020年11月于我院接受腹腔镜阑尾切除术的200例化脓性阑尾炎患者作为研究对象,根据保守治疗后实施腹腔镜阑尾切除术时机的不同将其分为参照组(93例)和研究组(107例)。参照组手术时间距保守治疗>72 h,研究组手术时间距保守治疗≤72 h。比较两组的治疗效果。结果术前及术后72 h,两组的细胞间黏附分子-1(ICAM-1)、基质金属蛋白酶-9(MMP-9)水平无显著差异(P>0.05);术后72 h,两组的ICAM-1、MMP-9水平均显著高于术前,差异具有统计学意义(P<0.05)。两组的手术时间、术后排气时间及术后抗生素使用时间无显著差异(P>0.05);研究组的住院时间短于参照组,治疗费用少于参照组,差异具有统计学意义(P<0.05)。研究组的并发症总发生率略高于参照组,但差异无统计学意义(P>0.05)。结论尽早为化脓性阑尾炎患者实施手术治疗能更好地帮助其缩短住院时间、节约治疗费用,相较于保守治疗72 h后再行手术治疗利大于弊。
Objective To compare the clinical effects of laparoscopic appendectomy at different times after conservative treatment in the treatment of suppurative appendicitis.Methods A total of 200 patients with suppurative appendicitis who underwent laparoscopic appendectomy in our hospital from August 2014 to November 2020 were selected as the research objects.According to the different times of laparoscopic appendectomy after conservative treatment,the patients were divided into reference group(93 cases)and study group(107 cases).The operation time of the reference group was>72 h from conservative treatment,and the operation time of the study group was≤72 h from conservative treatment.The therapeutic effects of the two groups were compared.Results There were no significant differences in the levels of intercellular adhesion molecule-1(ICAM-1)and matrix metalloproteinase-9(MMP-9)between the two groups before and 72 h after operation(P>0.05);at 72 h after operation,the levels of ICAM-1 and MMP-9 in the two groups were significantly higher than those before operation,and the differences were statistically significant(P<0.05).There were no significant differences in operation time,postoperative exhaust time and the postoperative antibiotic use time between the two groups(P>0.05);the hospital stay in the study group was shorter than that in the reference group,the treatment cost was less than that in the reference group,and the differences were statistically significant(P<0.05).The total incidence of complications in the study group was slightly higher than that in the reference group,but the difference was not statistically significant(P>0.05).Conclusion Early surgical treatment for patients with suppurative appendicitis can better help to shorten hospital stay and save treatment costs.Compared with conservative treatment after 72 h,early surgical treatment has more advantages than disadvantages.
