摘要
目的 探讨胃癌组织中circ0076704、人表皮生长因子受体2(HER2)、细胞间黏附分子-1(ICAM-1)的表达及意义.方法 选取50例胃癌患者,术中采集患者的胃癌组织和癌旁正常胃黏膜组织标本,荧光实时定量PCR检测组织标本中circ0076704表达;免疫组化法检测组织标本中HER2、ICAM-1表达.收集患者的人口学特征、肿瘤部位、组织分化程度、肿瘤直径、浸润深度、淋巴结转移情况、远处转移情况、幽门螺杆菌感染情况.从纳入本研究起对患者进行3 a的随访,记录其生存情况.结果 胃癌组织中circ0076704相对表达量及HER2阳性表达率明显高于正常胃组织,ICAM-1阳性表达率明显低于正常胃组织(P<0.01).胃癌组织中circ0076704相对表达量、HER2、ICAM-1阳性表达率在不同浸润深度、淋巴结转移、远处转移的胃癌患者间比较差异均具有统计学意义(P<0.01).ROC曲线分析显示,circ0076704、HER2、ICAM-1单独和联合检测在胃癌患者预后评估中具有较高的应用价值(P<0.01),其中联合检测的诊断价值最高(AUC=0.958),灵敏度、阳性预测值高于各指标单独检测.50例胃癌患者3 a生存率为52.00%(26/50).血清circ0076704≥22.37 U/L、HER2阳性、ICAM-1阴性的胃癌患者3 a生存率明显低于血清circ0076704<22.37 U/L、HER2阴性、ICAM-1阳性的胃癌患者(Log-rank χ2值分别为26.392、14.681、17.045,均P<0.01).结论 胃癌组织中circ0076704、HER2高表达,ICAM-1则低表达;联合检测对胃癌患者预后具有较高的评估价值.
Objective To investigate the expression and significance of circ0076704,HER2,ICAM-1 in gastric cancer.Method 50 patients with gastric cancer were selected.During the operation,the gastric cancer tissues and normal gastric mucosa tissues adjacent to the cancer were collected,and the expression of circ0076704 was detected by real-time quantitative PCR;The expression of HER2 and ICAM-1 was detected by immunohisto-chemistry.Collect the demographic characteristics,tumor location,tissue differentiation,tumor diameter,depth of invasion,lymph node metastasis,distant metastasis,and Helicobacter pylori infection of patients.The patients were followed up for 3 years since they were included in this study to record their survival.Results The relative expression of circ0076704 and the positive rate of HER2 in gastric cancer tissues were significantly higher than those in normal gastric tissues,and the positive rate of ICAM-1 was significantly lower than that in normal gastric tissues(P<0.01).The relative expression of circ0076704 in gastric cancer tissues was significantly different among gastric cancer patients with different invasion depth,lymph node metastasis and distant metastasis(P<0.01).The positive expression rates of HER2 and ICAM-1 in gastric cancer tissues were significantly different among gastric cancer patients with different invasion depth,lymph node metastasis and distant metastasis(P<0.01).ROC curve analysis showed that circ0076704,HER2,ICAM-1 alone and jointly had higher application value in the prognosis evaluation of gastric cancer patients(P<0.01);The diagnostic value of combined detection was the highest(AUC=0.958),and the sensitivity and positive predictive value were higher than those of individual detection.The 3-year survival rate of 50 patients with gastric cancer was 52.00%(26/50).The 3-year survival rate of gastric cancer patients with serum circ0076704≥22.37 U/L,HER2 positive and ICAM-1 negative was significantly lower than that of gastric cancer patients with serum circ0076704<22.37 U/L,HER2 negative and ICAM-1 positive(Log-rankχ2 values were 26.392,14.681,17.045,respectively P<0.01).Conclusion The expression of circ0076704 and HER2 is high,but ICAM-1 is low in gastric cancer;The combined detection has a high evaluation value for the prognosis of gastric cancer patients.
