生物电治疗女性盆底功能障碍性疾病对盆底肌电生理、四维超声参数及血清指标的影响 被引量:11

摘要 目的:研究生物电治疗女性盆底功能障碍性疾病对盆底肌电生理、四维超声参数及血清指标的影响。方法:选择2020年5月-2021年12月赣南医学院第一附属医院收治的100例女性盆底功能障碍性疾病患者进行前瞻性研究,随机分为观察组和对照组,各50例。观察组接受生物电治疗联合盆底肌训练,对照组接受盆底肌训练。两组在治疗前及治疗后4周进行盆腔脏器脱垂/尿失禁性功能问卷(PISQ-12)评分,检测盆底肌电生理指标(快肌最大肌电值、慢肌平均肌电值、混合肌平均肌电值)、盆底四维超声参数(静息状态和缩肛状态的肛提肌裂孔面积、肛提肌厚度)及血清基质金属蛋白酶-1(MMP-1)、骨桥蛋白(OPN)、结缔组织生长因子(CTGF)的含量。结果:治疗后,观察组ICIQ-SF评分低于对照组,PISQ-12评分高于对照组(P<0.05);治疗后,观察组盆底肌快肌最大肌电值、慢肌平均肌电值、混合肌平均肌电值均高于对照组(P<0.05);治疗后,观察组静息、缩肛状态的肛提肌裂孔面积均小于对照组,提肛肌厚度均大于对照组(P<0.05);治疗后,观察组血清MMP-1、OPN、CTGF含量均低于对照组(P<0.05)。结论:生物电治疗能够减轻女性盆底功能障碍性疾病症状,提高盆底肌电值和肛提肌厚度,促进肛提肌裂孔恢复,下调MMP1、OPN、CTGF。 Objective: To study the effects of bioelectric therapy on the electrophysiology of pelvic floor muscle, four dimensional ultrasound parameters and serum indexes of female pelvic floor dysfunction. Method: A total of 100 female patients with pelvic floor dysfunction admitted to the First Affiliated Hospital of Gannan Medical University from May 2020 to December 2021 were selected for a prospective study, they were randomly divided into observation group and control group, 50 cases in each group. The observation group received bioelectric therapy combined with pelvic floor muscle training, and the control group received pelvic floor muscle training.Before treatment and 4 weeks after treatment, the two groups received scores from pelvic organ prolapse/urinary incontinence functional questionnaire(PISQ-12). The electrophysiological parameters of pelvic floor(maximum electromyography value of fast muscle, average electromyography value of slow muscle, average electromyography value of mixed muscle), four-dimension ultrasonic parameters of pelvic floor(hiatus area of levator muscle in resting state and anal contraction state, thickness of levator muscle), serum matrix metalloproteinase-1(MMP-1),osteopontin(OPN) and connective tissue growth factor(CTGF) were detected. Result: After treatment, ICIQ-SF score of observation group was lower than that of the control group, PISQ-12 score was higher than that of the control group(P<0.05). After treatment, the maximum EMG value of fast muscle, average EMG value of slow muscle and average EMG value of mixed muscle of pelvic floor muscle in observation group were higher than those in the control group(P<0.05). After treatment, the hiatus area of levator anal muscle in resting and retraction state in observation group was smaller than that in the control group, and the thickness of levator anal muscle was larger than that in the control group(P<0.05). After treatment, the contents of MMP-1, OPN and CTGF in serum in observation group were lower than those in the control group(P<0.05). Conclusion: Bioelectric therapy can reduce the symptoms of pelvic floor dysfunction in women, improve the electric value of pelvic floor and the thickness of levator ANI muscle,promote the recovery of hiatus of levator ANI muscle, and down-regulate MMP1, OPN and CTGF.
