血清基质金属蛋白酶-1、骨桥蛋白、结缔组织生长因子水平与子宫切除术后患者盆底功能障碍发生的相关性研究 被引量:13

The relationship between serum MMP-1,OPN and CTGF level and pelvic floor dysfunction after hysterectomy

摘要 目的探讨血清基质金属蛋白酶-1(MMP-1)、骨桥蛋白(OPN)、结缔组织生长因子(CTGF)水平与子宫切除术后患者盆底功能障碍(PFD)发生的关系。方法选取2019年1月至2020年6月在重庆市壁山区中医院行子宫切除术后发生PFD的53例患者纳入PFD组,根据盆腔脏器脱垂程度分为Ⅰ度组15例、Ⅱ度组21例、Ⅲ度组17例,根据压力性尿失禁程度分为Ⅰ度组16例、Ⅱ度组23例、Ⅲ度组14例。另选择同期行子宫切除术后未发生PFD的204例患者纳入对照组。检测血清MMP-1、OPN、CTGF水平,分析MMP-1、OPN、CTGF水平与子宫切除术后PFD的关系,以及MMP-1、OPN、CTGF预测子宫切除术后发生PFD的价值。结果PFD组血清MMP-1、OPN、CTGF水平高于对照组(P<0.05),盆腔脏器脱垂、压力性尿失禁Ⅲ度患者血清MMP-1、OPN、CTGF水平高于Ⅱ度和Ⅰ度,Ⅱ度高于Ⅰ度(P<0.05)。MMP-1(OR=1.403,95%CI:1.398~1.574)、OPN(OR=1.642,95%CI:1.501~1.697)、CTGF(OR=1.501,95%CI:1.423~1.685)与子宫切除术后PFD的发生有关(P<0.05)。联合MMP-1、OPN、CTGF预测子宫切除术后发生PFD发生的AUC为0.908,高于MMP-1、OPN、CTGF单独的0.767、0.776、0.753(P<0.05)。结论子宫切除术后PFD患者血清MMP-1、OPN、CTGF水平均升高,MMP-1、OPN、CTGF水平与PFD发生和病情程度均有关,可作为子宫切除术后PFD发生的评估指标。 Objective To investigate the relationship between serum matrix metalloproteinase-1(MMP-1),osteopontin(OPN),and connective tissue growth factor(CTGF)level and pelvic floor dysfunction(PFD)after hysterectomy.Methods 53 patients with PFD after hysterectomy in Chongqing Bishan Hospital of Traditional Chinese Medicine from January 2019 to June 2020 were included in the PFD group.According to the extent of pelvic organ prolapse,PFD patients were divided into Ⅰ degree group(15 cases),Ⅱ degree group(21 cases),Ⅲ degree group(17 cases).According to the stress urinary incontinence,Ⅰ degree group(16 cases),Ⅱ degree group(23 cases),Ⅲ degree group(14 cases).In addition,204 patients without PFD after hysterectomy were selected as the control group.Serum level of MMP-1,OPN and CTGF were detected in all subjects.The relationship between the levels of MMP-1,OPN and CTGF and PFD after hysterectomy and the value of MMP-1,OPN and CTGF in predicting the occurrence of PFD after hysterectomy was analyzed.Results The serum level of MMP-1,OPN and CTGF in PFD group were higher than those in the control group(P<0.05).Serum level of MMP-1,OPN,CTGF in Uterine prolapse and stress urinary incontinence Ⅲ degree were higher than Ⅱ and Ⅰ degree(P<0.05),and that in uterine prolapse and stress urinary incontinence Ⅱ degree were higher than Ⅰ degree(P<0.05).MMP-1(OR=1.403,95%CI:1.398~1.574),OPN(OR=1.642,95%CI:1.501~1.697),and CTGF(OR=1.501,95%CI:1.423~1.685)were still correlated with the occurrence of PFD after hysterectomy(P<0.05).The AUC of combined MMP-1,OPN and CTGF in predicting the occurrence of PFD after hysterectomy was 0.908,which was higher than that of MMP-1,OPN and CTGF alone(0.767,0.776 and 0.753,P<0.05).Conclusions Serum level of MMP-1,OPN and CTGF are all increased in PFD patients after hysterectomy,and the level of MMP-1,OPN and CTGF are related to the occurrence and severity of PFD,and could be used as an evaluation index for the occurrence of PFD after hysterectomy.
