摘要
[目的]探讨范永升教授运用补肾滋阴、涤痰开窍法治疗系统性红斑狼疮脑病(systemic lupus erythematosus encephalopathy,SLEE)的临床经验。[方法]通过临证学习和查阅SLEE的病因病机和辨证论治,阐述范永升教授治疗SLEE的临床经验,并附医案一则。[结果]范永升教授认为,SLEE的主要病因病机以肾阴不足为本,风痰上扰清窍为标。在临床诊疗中应根据患者临床症状和体征判别疾病的轻重和缓解程度,在补肾滋阴或涤痰开窍上有所侧重,兼顾狼疮本病和继发的神经精神系统损害,动态观察疾病的发展过程。在本例医案中,范永升教授运用补肾滋阴、涤痰开窍法治疗SLEE,有效改善了患者的临床症状和实验室指标。[结论]范永升教授运用补肾滋阴、涤痰开窍法治疗SLEE,理论依据充分,临床疗效显著,值得推广学习。
[Objective]To explore Professor FAN Yongsheng's clinical experience in the treatment of systemic lupus erythematosus encephalopathy(SLEE)by tonifying the kidney and nourishing Yin,removing phlegm and opening orifices.[Methods]Through clinical study and access to the etiology and pathogenesis and syndrome differentiation and treatment of SLEE,Professor FAN Yongsheng's clinical experience in treating SLEE was elaborated,and a medical case was attached.[Results]Professor FAN Yongsheng believes that the main etiology and pathogenesis of SLEE is that the deficiency of kidney Yin is the base,and the disturbance of wind and phlegm is the superficial.Therefore,in clinical diagnosis and treatment,doctors should distinguish the severity and relief of the disease according to the clinical manifestations and signs of the patients,focusing on tonifying the kidney and nourishing Yin,removing phlegm and opening orifices.In the treatment,doctors should give consideration to the disease of SLE and the secondary damage of nervous and mental system,and dynamically observe the development process of the disease.In this case,Professor FAN Yongsheng used the methods of tonifying the kidney and nourishing Yin,removing phlegm and opening orifices to treat SLEE,which had a definite effect and effectively relived the clinical symptoms and improved laboratory indexes of the patient.[Conclusion]Professor FAN Yongsheng's treatment of SLEE by tonifying the kidney and nourishing Yin,removing phlegm and opening orifices has sufficient theoretical basis and remarkable clinical effect,which is worth popularizing and learning.
