摘要
目的探讨孕三烯酮联合醋酸曲普瑞林在子宫内膜异位症术后患者中的应用效果。方法将2018年3月至2020年3月在我院行腹腔镜手术治疗的66例子宫内膜异位症患者采用双盲法分为单一组(33例,醋酸曲普瑞林)与联合组(33例,孕三烯酮联合醋酸曲普瑞林)。比较两组的临床疗效、抗苗勒氏管激素(AMH)、抗子宫内膜抗体(EMAb)、糖类抗原125(CA125)、雌二醇(E_(2))、卵泡刺激素(FSH)、促黄体生成素(LH)水平、不良反应发生情况、复发情况、血管内皮生长因子(VEGF)、血管内皮生长因子受体-1(VEGFR-1)、血管内皮生长因子受体-2(VEGFR-2)水平。结果联合组的治疗总有效率为93.94%,高于单一组的75.76%(P<0.05)。治疗后,两组的AMH水平均升高,EMAb、CA125、E_(2)、FSH、LH水平均降低,且联合组优于单一组(P<0.05)。两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。联合组的复发率为3.03%,低于单一组的18.18%(P<0.05)。治疗后,两组的VEGF、VEGFR-1、VEGFR-2水平均降低,且联合组低于单一组(P<0.05)。结论孕三烯酮联合醋酸曲普瑞林治疗子宫内膜异位症术后患者的效果显著,可降低EMAb、CA125、E_(2)、FSH、LH、VEGF、VEGFR-1、VEGFR-2水平,缓解痛经,降低复发风险,安全性良好。
Objective To investigate the application effect of gestrinone combined with triptorelin acetate in patients with endometriosis after operation.Methods A total of 66 patients with endometriosis who underwent laparoscopic surgery in our hospital from March 2018 to March 2020 were divided into single group(33 cases,triptorelin acetate)and combined group(33 cases,gestrinone combined with triptorelin acetate)by double blind method.The clinical efficacy,anti-Müllerian hormone(AMH),endometrial antibody(EMAb),carbohydrate antigen 125(CA125),estradiol(E_(2)),follicle stimulating hormone(FSH),luteinizing hormone(LH)levels,occurrence of adverse reactions,recurrence situation,vascular endothelial growth factor(VEGF),vascular endothelial growth factor receptor-1(VEGFR-1),and vascular endothelial growth factor receptor-2(VEGFR-2)levels were compared between the two groups.Results The total effective rate of treatment in the combined group was 93.94%,which was higher than 75.76%in the single group(P<0.05).After treatment,the level of AMH in the two groups increased,and the levels of EMAb,CA125,E_(2),FSH and LH decreased,and those in the combined group were better than the single group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).The recurrence rate in the combined group was 3.03%,which was lower than 18.18%in the single group(P<0.05).After treatment,the levels of VEGF,VEGFR-1 and VEGFR-2 in the two groups decreased,and those in the combined group were lower than the single group(P<0.05).Conclusion Gestrinone combined with triptorelin acetate in the treatment of patients with endometriosis after operation has significant effect,which can reduce the levels of EMAb,CA125,E_(2),FSH,LH,VEGF,VEGFR-1 and VEGFR-2,alleviate the dysmenorrhea,reduce the risk of recurrence,and has good safety.
