摘要
目的探讨胃癌患者血清T淋巴细胞亚群水平与化疗效果的相关性。方法抽取苏州大学附属常熟医院2017年6月至2020年5月收治的89例胃癌患者,所有患者均接受化疗,并于治疗4个月后,根据世界卫生组织(WHO)实体瘤客观疗效评价标准评估效果,所有患者进行血清T淋巴细胞亚群水平检测,分析血清T淋巴细胞亚群水平与化疗效果的相关性。结果入选的89例胃癌患者,有效64例,占比71.91%,纳入有效组;无效组25例,占比28.09%。有效组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平高于无效组,CD8^(+)水平低于无效组(P<0.05)。绘制受试者工作特征曲线发现,血清CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)、CD8^(+)预测胃癌化疗无效的曲线下面积(AUC)分别为0.930、0.898、0.757、0.816。结论T淋巴细胞亚群和胃癌化疗效果有密切关系,临床可于胃癌患者化疗期间实时检测T淋巴细胞亚群水平,并对水平异常患者进行干预,以达到改善疗效作用。
Objective To investigate the correlation between serum T lymphocyte subsets and chemotherapy effect in patients with gastric cancer.Methods Eighty-nine cases of gastric cancer patients in Changshu Hospital Affiliated to Soochow University from June 2017 to May 2020 were selected.All patients received chemotherapy.And after 4 months of treatment,the effect was evaluated according to the World Health Organization(WHO)objective curative effect evaluation standard for solid tumors.All patients were tested for the level of serum T lymphocyte subsets to analyze the correlation between the serum T lymphocyte subsets level and the effect of chemotherapy.Result Among the 89 patients with gastric cancer,64 cases were in effective group,accounting for 71.91%;25 cases were in ineffective group,accounting for 28.09%;the levels of CD3^(+),CD4^(+),CD4^(+)/CD8^(+)in effective group were higher than those in ineffective group,and CD8^(+)level was lower than that in ineffective group(P<0.05);Draw the receiver operating characteristic curve and find that the area under the curve(AUC)of serum CD3^(+),CD4^(+),CD4^(+)/CD8^(+),CD8^(+)predicting the ineffectiveness of gastric cancer chemotherapy was 0.930,0.898,0.757,0.816,respectively.Conclusions T lymphocyte subsets are closely related to the effect of gastric cancer chemotherapy.Clinically,the level of T lymphocyte subsets in patients with gastric cancer can be detected in real time during chemotherapy,and patients with abnormal levels can be intervened to improve the efficacy.
