摘要
目的:调查70岁及以上住院患者衰弱现状,分析其影响因素。方法:随机选取2019年5-12月就诊于锦州医科大学附属第三医院干诊科70岁及以上住院患者200例为调查对象。采用一般资料问卷、FRAIL量表评估衰弱、日常生活活动能力调查表(ADL)调查失能,综合医院焦虑抑郁量表(HADS)评估焦虑抑郁状态,简易精神状态评估量表(MMSE)评估认知状态,微营养评估量表(MNA-SF)评估营养情况;采用二元logistic回归分析分析70岁及以上住院患者的影响因素。结果:共发放调查问卷200份,收回193份,有效率96.5%。其中61例符合衰弱标准,其发生率为31.6%。单因素分析显示,衰弱与非衰弱患者年龄、婚姻、居住、收入、文化程度、运动、视力障碍、听力障碍、共病、用药数量、认知功能比较,差异均有统计学意义(P<0.05)。多因素logistic回归分析结果显示,年龄、收入、用物数量、共病、认知功能均是住院患者衰弱的影响因素(P<0.05)。结论:70岁及以上住院患者衰弱发生率高,且受多种因素影响。
Objective:To investigate the frailty status of hospitalized patients aged 70 years and above and analyze their influencing factors.Method:A total of 200 inpatients aged 70 years and above who visited the Department of Ganzhen in the Third Affiliated Hospital of Jinzhou Medical University from May to December 2019 were randomly selected as the subjects.FRAIL scale was used to assess weakness,daily life activity ability questionnaire (ADL) was used to investigate disability,general hospital anxiety depression scale (HADS) was used to evaluate anxiety and depression,MMSE was used to evaluate mental state,MNA-SF was used to assessed nutritional status,and binary logistic regression analysis analyzed the influencing factors of hospitalized patients aged 70 years and over.Result:A total of 200 questionnaires were distributed and 193 were recovered,the effective rate was 96.5%.Among them,61 cases met the standard of asthenia,and the incidence rate was 31.6%.Univariate analysis showed that there were significant differences in age,marriage,residence,income,education level,sports,visual impairment,hearing impairment,comorbidity,drug number and cognitive function between two groups (P<0.05).Multifactor logistic regression analysis showed that age,income,drug number,comorbidity and cognitive function were all contributing factors of weakness in inpatients (P<0.05).Conclusion:The incidence of frailty in hospitalized patients aged 70 years and above is high,and it is affected by many factors.
