摘要
目的:调查遂宁地区体检健康成人不同血浆渗透压计算公式的参考区间,为其建立适宜的参考区间提供数据支撑。方法:参照卫生行业标准WS/T 402-2012等文献资料制订参考个体的纳入和排除标准,从遂宁市中心医院募集20~79岁的体检健康成人,检测其肝素钠抗凝血浆钠、钾、尿素、血糖的浓度水平,然后运用5个临床常用的血浆渗透压计算公式计算出参考个体对应的血浆渗透压。使用Dixon检验剔除血浆渗透压离群值后,以非参数方法建立遂宁地区体检健康成人不同血浆渗透压计算公式的参考区间(P 2.5~P 97.5)。结果:共4802例体检健康成人被纳入研究,其中男2303例,女2499例。不同计算公式间血浆渗透压的差异均有统计学意义(P<0.01);各计算公式同一年龄段男性与女性血浆渗透压比较差异均有统计学意义(P<0.05);而相同性别不同年龄段间比较,仅有部分年龄段间浓度水平比较差异无统计学意义(P>0.05),5个计算公式中男性和女性血浆渗透压的浓度水平随年龄段的变化情况一致。不同血浆渗透压计算公式的参考区间为:公式1为273.64~289.34 mOsm/kg(H_(2)O);公式2为283.86~300.58 mOsm/kg(H_(2)O);公式3为291.48~308.44 mOsm/kg(H_(2)O);公式4为281.20~297.30 mOsm/kg(H_(2)O);公式5为281.70~297.65 mOsm/kg(H_(2)O)。遂宁地区体检健康成人不同血浆渗透压计算公式的参考区间上限均低于临床常用医学教科书或工具书中相应计算公式参考区间的上限,而参考区间的下限也与其存在差异。结论:遂宁地区体检健康成人血浆渗透压的参考区间受计算公式、性别和年龄的影响,因此有必要在合理分组后根据不同的计算公式来建立适宜于该地区人群的参考区间,以更好地服务临床。
Objective:To investigate the reference interval of different plasma osmotic pressure calculation formula in Suining,and provides data support for its establishment of appropriate reference interval.Methods:Reference to documents such as the Health Industry Standard WS/T 402-2012 make reference to the individual's inclusion criteria and exclusion criteria,from Suining Central Hospital physical examination department.crowd in the raise 20~79 years old medical health adult,detecting the heparin anticoagulant blood plasma sodium,potassium,urea,blood sugar levels,then using five clinical commonly used plasma osmotic pressure calculation formula to calculate the corresponding reference individual plasma osmotic pressure.After the plasma osmotic pressure outliers were eliminated by Dixon test,the reference interval(P 2.5~P 97.5)of different plasma osmotic pressure calculation formulas for healthy adults in Suining area were established by non-parametric method.Results:A total of 4802 healthy adults were included in the study,including 2303 male and 2499 female reference subjects.The difference of plasma osmotic pressure between different calculation formulas was statistically significant(P<0.01),the difference of plasma osmotic pressure between male and female at the same age was statistically significant(P<0.05).However,in comparison between the same gender and different age groups,only part of the concentration levels between age groups showed no statistical significance(P>0.05),and the changes of plasma osmotic pressure concentration levels of male and female were consistent with the changes of age groups in the 5 calculation formulas.the difference in concentration level between each age group was consistent among the five calculation formulas,and only some age groups showed no statistical significance(P>0.05).The reference intervals of different plasma osmotic pressure calculation formulas are as follows:1.(273.64~289.34)mOsm/kg(H_(2)O),2.(283.86~300.58)mOsm/kg(H_(2)O),3.(291.48~308.44)mOsm/kg(H_(2)O),4.(281.20~297.30)mOsm/kg(H_(2)O),5.(281.70~297.65)mOsm/kg(H_(2)O).The upper limit of reference interval of different plasma osmotic pressure calculation formulas for healthy adults in Suining is lower than the upper limit of reference interval of corresponding calculation formulas in clinical medical textbooks or reference books,and the lower limit of reference interval is also different from that of healthy adults in Suining.Conclusion:Suining area physical healthy adults plasma osmotic pressure reference range from calculation formula and the influence of gender and age,so it is necessary to reasonably after grouped according to different calculation formula to establish reference range,suitable for the region's people to a better service.
