摘要
目的测定北京某社区T2DM患者足背经皮氧分压(TcPO2)水平现状。方法选取2017年12月至2018年11月于北京市某社区卫生服务中心门诊连续就诊的T2DM无糖尿病足(DF)、年龄≥50岁患者313例,测量左右两侧足背TcPO2并收集患者临床资料。结果测量仰卧位平放双下肢足背TcPO2平均值为(55±15)mmHg;按照两侧足背TcPO2≥60 mmHg为正常,所有患者中250例(80%)TcPO2降低,其中TcPO251~59 mmHg、41~50 mmHg和≤40 mmHg患者分别为95例(30%)、87例(28%)和68例(22%)。结论社区≥50岁T2DM无DF患者足背TcPO2数值普遍低于正常水平,测定TcPO2能早期发现亚临床状态足部组织灌注不足,为社区T2DM患者防治DF提供新方法。
Objective To investigate the current status of transcutaneous oxygen pressure(TcPO2)in type 2 diabetes mellitus patients in a community at Beijing.Methods A total of 313 patients followed up in a community health service center in Beijing were enrolled in this study from December 2017 to Novem⁃ber 2018.All the patients were diagnosed with T2DM,aged≥50 years,and without diabetic foot.The TcPO2 were measured in both left and right sides.Clinical information was collected from all the patients.Results The mean value of TcPO2 of the lower extremity was(55±15)mmHg measured in the supine po⁃sition.Normal TcPO2 was≥60 mmHg on both sides of dorsum foot,and reduced TcPO2 was<60 mmHg on one side or both sides of dorsum foot.In all the patients,250 cases(80%)had reduced TcPO2.Among them,95 cases(30%),87 cases(28%)and 68 cases(22%)had TcPO2 value of 51~⁃59 mmHg,41~50 mmHg and<40 mmHg,respectively.Conclusion In Community,TcPO2 was reduced in most T2DM patients who are over 50 years old and without diabetic foot.TcPO2 testing could detect early subclinical foot tissue perfusion deficiency,suitable for application in community medical services,and could provide a new method for the prevention and treatment of diabetic foot in community type 2 diabetes mellitus patients.
