独活寄生汤联合针刺治疗腰椎间盘突出症的临床效果被引量：39

摘要目的:探讨独活寄生汤联合针刺治疗对腰椎间盘突出症患者疗效及其机制。方法:选取2016年1月至2018年1月在汕尾骨科医院接受治疗的腰椎间盘突出症患者98例进行回顾性分析,按照治疗方式的不同分为对照组和观察组,每组49例,对照组49例行西医常规治疗加以独活寄生汤治疗,观察组49例在此基础加以针刺治疗。比较2组疗效、血清疼痛物质、炎性反应递质及氧化应激反应。结果:对照组治疗有效率为83.67%,高于观察组的95.91%,差异有统计学意义(P<0.05)。2组治疗后P物质、神经肽、神经生长因子水平均明显下降,但观察组下降更为明显,差异有统计学意义(P<0.05)。治疗后2组白细胞介素-1β、白细胞介素-6、超敏C反应蛋白水平均明显下降,但观察组下降更为明显,差异有统计学意义(P<0.05)。治疗后2组超氧岐化酶、谷胱甘肽水平均明显升高,但观察组升高更明显,血清丙二醛水平明显下降,但观察组下降更显著,差异有统计学意义(P<0.05)。结论:独活寄生汤联合针刺治疗较单独使用独活寄生汤可明显提高腰椎间盘突出症患者疗效,降低血清疼痛物质及炎性反应递质水平,改善氧化应激反应。 Objective:To explore the effects and mechanism of Duhuozhitang Decoction combined with acupuncture on patients with lumbar disc herniation.Methods:A retrospective analysis was performed on 98 patients with lumbar disc herniation who were treated in Shanwei Orthopaedic Hospital from January 2016 to January 2018.The patients were divided into 2 groups according to the different treatment methods.The control group was treated with conventional Western medicine and Duhuo Jisheng Decoction,and 49 cases in the observation group was treated with acupuncture based on the control group.The curative effects,serum pain substances,inflammatory factors and oxidative stress were compared between the 2 groups.Results:The effective rate of the control group was 83.67%,which was significantly higher than that of the observation group(95.91%)(P<0.05).After treatment,the levels of substance P,neuropeptide and nerve growth factor decreased significantly in both groups,but the decrease was more obvious in the observation group(P<0.05).After treatment,the levels of interleukin-1β,interleukin-6 and hypersensitive C-reactive protein in the 2 groups were decreased significantly,but the decrease in the observation group was more obvious(P<0.05).After treatment,the levels of superoxide dismutase and glutathione in both groups were significantly increased,but those in the observation group increased significantly.The levels of serum malondialdehyde were significantly decreased,but in the observation group,those were significantly increased(P<0.05).Conclusion:Duhuo Jisheng Decoction combined with acupuncture can significantly improve the curative effects of patients with lumbar disc herniation,reduce the levels of pain substances and inflammatory factors in serum,and improve oxidative stress response.
