摘要
[目的]设计消化内镜治疗围术期规范化护理落实单并应用于临床。[方法]参考相关指南和文献并结合临床实际设计消化内镜治疗围术期规范化护理落实单,将其应用于择期内镜治疗患者,比较落实单使用前(使用前组)与落实单使用后(使用后组)的消化内镜治疗亚专科护理综合质量评分、亚专科护理质量监测指标结果。[结果]使用后组亚专科护理综合质量评分中护理评估、患者准备、健康教育、病情观察、护理措施、操作技术、出院随访7个维度的单项得分和总分较使用前组有明显提高,差异有统计学意义(t=8.135~20.571,均P<0.01);使用后组亚专科护理质量监测指标中,4级内镜治疗患者术前准备合格率提高、护士对患者病情动态掌握达标率提高、内镜治疗相关迟发性出血发生率下降、出院患者满意度提高,与使用前组比较,差异有统计学意义(χ~2=7.939~10.902,t=3.239,均P<0.01)。[结论]应用消化内镜治疗围术期规范化护理落实单,有助于规范护理临床实践和质量监控行为,有效提高内镜治疗亚专科护理工作质量,保障患者的安全。
[Objective]To design and apply a standardized nursing implementation table in perioperative period of digestive endoscopic therapy.[Methods]A standardized nursing implementation table in perioperative period of digestive endoscopic therapy was designed by referring to guidelines and related literatures,combining with the clinical practice.The implementation table was used in patients treated with selective endoscopic therapy from June 2018 to May 2019.Observation and comparison were conducted in the subspecialty nursing comprehensive quality score,the outcome of subspecialty nursing quality monitoring indicators before and after application of the table.[Results]After the application of the table,in the subspecialty nursing comprehensive quality score,the individual score and total scores of 7 dimensions including nursing evaluation,patient preparation,health education,condition observation,nursing measures,operating techniques and discharge follow-up,were significantly improved compared with those before the application,there were significant differences(t=8.135-20.571,all P<0.01).In the outcome of subspecialty nursing quality monitoring indicators,the qualified rate of preoperative preparation of patients with grade Ⅳ endoscopic therapy was improved,the compliance rate of nurse grasp the dynamic state of the patient’s condition was improved,the incidence of delayed hemorrhage associated with endoscopic therapy decreased,the satisfaction degree of discharged patients increased,and compared with those before the application,there were significant differences(χ~2=7.939-10.902,t=3.239,all P<0.01).[Conclusion]The application of the standardized nursing implementation table helped to standardize the nursing clinical practice and quality monitoring behavior,improve the quality of subspecialty nursing work of endoscopic therapy effectively,and ensure patient safety.
