摘要
目的探讨基于营养管理的饮食干预对糖尿病患者血糖、血脂水平的影响。方法回顾性分析2014年6月至2015年6月期间收治的糖尿病患者100例,依据干预措施不同将其分为营养管理组与常规组各50例。常规组给予常规健康教育、生活及饮食指导,而营养管理组在此基础上给予基于营养管理的饮食干预。统计分析两组患者服药依从性、糖尿病饮食知识掌握程度、自我管理能力、血糖及血脂水平情况。结果营养管理组服药总依从率高于常规组,差异有统计学意义(P<0.05);营养管理组糖尿病饮食知识掌握率与常规组相比,无明显差异(P>0.05);营养管理组饮食管理、健康生活方式、心理及社会适应、定期复查评分均明显高于常规组,差异有统计学意义(P<0.05);两组患者管理后空腹血糖(FPG)、餐后2 h血糖(2hPG)、糖化血红蛋白(HbAlc)、甘油三酯(TG)、总胆固醇(TC)及低密度脂蛋白(LDL-C)水平明显低于管理前,HDL-C水平管理后高于管理前,差异有统计学意义(P<0.05);营养管理组管理后FPG、2hPG、HbAlc、TG、TC及LDL-C水平降低程度明显低于常规组,而HDL-C水平升高程度高于常规组,差异有统计学意义(P<0.05);logistic回归性分析结果显示,糖尿病饮食知识掌握程度、服药依从性及自我管理能力升高是糖尿病患者血糖、血脂控制良好的独立影响因素(P<0.05)。结论基于营养管理的饮食干预能有效提高糖尿病患者服药依从性及自我管理能力,有效控制其血糖、血脂水平。
Objective To explore the effects of diet intervention on the levels of blood glucose and lipid in the patients with diabetes.Methods A total of 100 patients of diabetes were collected from June 2014 through to June 2015 in the retrospective analysis.According to the different interventions,a nutrition management group and a routine treatment group were divided,50 cases in each one.In the routine treatment group,the health education,life and diet guidance were provided.In the nutrition management group,on the base of the treatment as the routine treatment group,the diet intervention was given.The medication compliance,the awareness degree of diet knowledge of diabetes,self-management ability and the levels of blood glucose and lipid were analyzed statistically.Results The total compliance rate of medication in the nutrition management group was higher than the routine treatment group,indicating the significant difference(P<0.05).The difference was not significant statistically in the awareness degree of diet knowledge of diabetes between the two groups(P>0.05).In the nutrition management group,the scores of diet management,healthy life style,psycho-physical adaptability and routine examination were all obviously higher than the routine treatment group,indicating the significant differences(P<0.05).In both of the two groups,the levels of FPG,2hPG,HbAlc,TG,TC and LDL-C after treatment were all lower obviously than those before treatment and HDL-C level was higher than that before treatment,indicating the significant differences(P<0.05).,After treatment,the levels of FPG,2hPG,HbAlc,TG,TC and LDL-C after treatment In the nutrition management group were all lower obviously than and HDL-C level was higher than the routine treatment group(P<0.05).The logistic regression analysis results showed that the improvements of the medication compliance,the awareness degree of diet knowledge of diabetes and the self-management ability are the independent factors of the effective control of blood glucose and lipid in diabetes patients(P<0.05).Conclusion The diet intervention on the base of nutrition management effectively improves the medication compliance and self-management ability and controls blood glucose and lipid in diabetes patients.
