摘要
目的将3D打印技术应用于住院医师规范化培训的复杂创伤骨科病例临床教学中,探究教学效果。方法纳入64名2017年9月—2018年6月在四川大学华西医院骨科进行住院医师规范化培训的学员,以入科序号按照单双号随机分为实验教学组32人和传统教学组32人。对复杂创伤骨折病例使用CT三维重建并利用软件建模后,再应用3D打印机打印出模型用于实验教学组临床教学;而传统教学组学员采用病史采集、体格检查、查看影像学资料、教师讲授等方法进行临床教学。最后,通过出科考核和教学评价进行教学效果评估。结果理解理论知识、提高手术操作技能、提高学习兴趣、实践考核成绩、满意度等方面的评分,实验教学组均高于传统教学组,差异有统计学意义(P<0.05);两组理论考核成绩差异无统计学意义。结论3D打印技术在创伤骨科复杂病例教学中,可以帮助规培学员理解理论知识、提高手术操作技术,提高其学习兴趣、实践考核成绩和满意度。
Objective To apply 3D printing technology to the clinical teaching of complex traumatic orthopedics cases with standardized training of resident physicians and explore the teaching effective.Methods 64 students who had received standardized training of resident physicians in orthopedics department of West China Hospital of Sichuan University from September 2017 to June 2018 were enrolled,and were randomly divided into experimental teaching group ( ETG,n = 32 ) and traditional teaching group ( TTG,n = 32 ) according to even and odd numbers.After the threedimensional reconstruction of complex fractures with CT,the Mimics software was used to establish the three-dimensional model,and then the 3D printer was used to print out the complex fracture model for clinical teaching in ETG.The TTG adopted the methods of medical history collection, physical examination,study of imaging data,explanation of injury mechanism,clinical manifestations and treatment strategies for clinical teaching.Finally,the teaching effective valuated through the examination and teaching evaluation.Results The scores of the ETG in terms of understanding of the theoretical knowledge,improvement of surgical skills, improvement of learning interest,practical assessment and satisfaction was higher than ones of the TTG,and the difference was statistically significant (P < 0.05).There was no statistically significant difference in theoretical assessment scores between the two groups.Conclusion In the teaching of complex cases of traumatic orthopaedics,3D printing technology can help the trainees to understand theoretical knowledge,to improve surgical operation technology,to improve their learning interest,to improve their practical assessment score and satisfaction.
