摘要
目的:探讨健脾祛湿中药汤剂用于肝脾虚湿阻型非酒精性单纯性脂肪肝(nonalcoholicsimplefattyliver,NAFL)的临床疗效。方法:选取2015年1月至2017年3月中山市博爱医院收治的肝脾虚湿阻型NAFL患者90例,按照入组先后顺序采用奇偶数字表法分组,奇数为对照组,偶数为观察组,每组45例。对照组患者在常规治疗基础上口服肌醇片,观察组患者在常规治疗基础上口服健脾祛湿中药汤剂,比较两组患者治疗前后临床证候积分、临床疗效及肝功能相关指标水平的差异。结果:治疗后,观察组患者平均临床证候积分为(10.7±2.1)分,明显低于对照组的(14.8±2.9)分,差异有统计学意义(P<0.001);观察组患者的总有效为95.6%(43/45),明显高于对照组的71.1%(32/45),差异有统计学意义(P<0.05);治疗后,观察组患者的谷氨酰转肽酶、丙氨酸氨基转移酶及天门冬氨酸氨基转移酶水平明显低于对照组,差异均有统计学意义(P<0.001)。结论:健脾祛湿中药汤剂用于肝脾虚湿阻型NAFL,可更好地改善患者的临床症状,疗效优于西药治疗,其疗效可能与改善肝功能有关。
OBJECTIVE: To probe into the clinical efficacy of Jianpiqushi decoction in treatment of non-alcoholic simple fatty liver disease(NAFLD) of spleen deficiency and dampness. METHODS:90 patients with NAFLD of spleen deficiency and dampness admitted into Boai Hospital of Zhongshan from Jan. 2015 to Mar. 2017 were selected and sorted by admission sequence, the odd numbers were set as control group and even numbers were observation group, with 45 cases in each group. The control group was treated with Inositol tablets based on conventional therapy, while the observation group was given Jianpiqushi decoction based on conventional therapy, differences in clinical syndrome scores, efficacy and indices of liver function between two groups were compared. RESULTS: After treatment, the average clinical syndrome score of observation group was(10.7±2.1) points, which was significantly lower than that of the control group [(14.8±2.9) points], with statistically significant difference(P<0.001);the total effective rate of observation group was 95.6%(43/45), which was significantly higher than that of the control group(71.1%, 32/45), with statistically significant difference(P<0.05);after treatment, the glutamyl transpeptidase,alanine aminotransferase and aspartate aminotransferase levels of observation group were significantly lower than those of the control group, with statistically significant difference(P<0.001). CONCLUSIONS: The application of Jianpiqushi decoction in treatment of NAFLD of spleen deficiency and dampness can effectively improve clinical symptoms, with better efficacy than western medicine, which may be related to its improvement on liver function.
