乳腺癌易感基因1在散发性浸润性乳腺癌中的表达及其意义 被引量:9

摘要 目的乳腺癌易感基因1(BRCA1)是乳腺癌最重要的易感基因之一。文中旨在探讨BRCA1在散发性浸润性乳腺癌中的表达情况及其与临床各指标的相关性。方法回顾性分析2015年1月至2017年12月南通市肿瘤医院病理科散发性浸润性乳腺癌标本618份。采用免疫组化方法检测标本中BRCA1、ER、PR、HER2及Ki67蛋白的表达,分析BRCA1与散发性浸润性乳腺癌分子分型、组织学类型及相关分子生物学标记物之间的关系。结果 BRCA1阳性率为44.2%,其中弱阳性(+)为30.3%,强阳性(++)为13.9%。ER、PR、HER2阳性率分别为60.8%、54.7%、24.9%。Ki67增殖指数≤30%为70.7%,>30%为29.3%。三阴型乳腺癌中BRCA1阳性表达率(36.97%)明显低于Luminal A型(45.86%)、HER2阴性Luminal B型(40.96%)、HER2阳性Luminal B型(59.32%)及HER2过表达型(46.24%)乳腺癌(P<0.05)。伴髓样特征的癌中BRCA1阳性表达率(21.01%)明显低于非特殊类型浸润性癌(46.52%)、浸润性小叶癌(27.27%)、黏液性癌(36.36%)及其他(34.29%)乳腺癌(P<0.05)。BRCA1和HER2在乳腺癌中的表达呈显著正相关(r=0.117,P<0.01)。结论 BRCA1在三阴型及伴髓样特征的乳腺癌表达下调,BRCA1可能影响散发性浸润性乳腺癌的发生发展过程。 Objective BRCA1 is one of the most important susceptibility genes of breast cancer. The article aimed to investigate the expression of BRCA1 and its correlation in sporadic invasive breast cancer. Methods The expressions of BRCA1,ER, PR,HER2 and Ki67 in 618 cases of sporadic invasive breast cancer in our hospital from January 2015 to December 2017 were detected with immunohistochemistry in order to investigate and analyze the expression of BRCA1 and its correlation with molecular classification, histological type and other related molecular markers in sporadic invasive breast cancer. Results The positive rate of BRCA1 was 44.2% consisting of 30.3% weak positive(+)and 13.9% strong positive(++). The positive rates of ER,PR,and HER2 are 60.8%, 54.7%,and 24.9%. The proliferation index ≤30% of Ki67 was 70.7%,>30% was 29.3%. The expression of BRCA1 in luminal type A was significantly lower than the other four types of sporadic invasive breast cancer(P<0.05). The expression of BRCA1 in breast cancer with medullary histological features was significantly lower than those of the other types of breast cancer(P<0.05). There was significant difference between the expression of BRCA1 and the expressions of ER,HER2 and Ki67(P<0.01). The expression of BRCA1 had positive correlation with expression of HER2 in sporadic invasive breast cancer(r=0.117,P<0.01). Conclusion The expression of BRCA1 in sporadic invasive breast cancer with triple negative subtype and medullary histological features is down-regulated and BRCA1 may affect the development and progression of sporadic invasive breast cancer.
