摘要 目的比较荧光原位杂交技术(FISH)与免疫组织化学法(IHC)用于检测HER-2的效果。方法将本院2020年1月-2022年3月期间收治的乳腺癌患者247例作为研究对象,均同时采用FISH与IHC检测HER-2基因或蛋白的表达情况,比较两种方法检测结果及一致性。结果本研究纳入的247例患者均留取合适标本进行相关检查,结果表明HER-2蛋白表达采用免疫组化检测+++,可疑及+患者分别有8.10%(20例),73.68%(182例)和10.12%(25例),HER-2蛋白表达采用荧光原位杂交技术检测有扩增和无扩增患者分别为30.36%(75例)和69.64%(172例)。247例患者免疫组化检测HER-2结果为+++、++和+时与荧光原位杂交技术检测的符合率分别为85.00%、96.70%、20.00%,免疫组化检测HER-2结果为+++、++的202例患者中有97.03%的患者FISH检测有HER-2基因扩增,而结果为+的患者中有20.00%患者FISH检测有HER-2基因扩增。FISH和IHC检测结果经一致性检验后发现一致性较好(Kappa值=0.543,P<0.05)。结论免疫组化和荧光原位杂交技术检测HER-2基因效果均较好,两者一致性较好,可在临床推广应用。 Objective To compare the effect of fluorescence in situ hybridization(FISH)and immunohistochemistry(IHC)on HER-2 detection.Methods 247 breast cancer patients admitted to our hospital from January 2020 to March 2022 were taken as the study subjects.Fish and IHC were used to detect the expression of HER-2 gene or protein at the same time,and the detection results and consistency of the two methods were compared.Results All 247 patients included in this study were taken suitable specimens for relevant examination,and the results showed that HER-2 protein expression was detected to be+++by immunohistochemistry,suspicious and+patients were 8.10%(20 cases),73.68%(182 cases)and 10.12%(25 cases)respectively.The expression of HER-2 protein was detected by fluorescence in situ hybridization in 30.36%(75 cases)and 69.64%(172 cases)of patients with and without amplification,respectively.The coincidence rates of HER-2 detection with fluorescence in situ hybridization were 85.00%,96.70%and 20.00%respectively when HER-2 results were+++,++and+by immunohistochemistry.97.03%of the 202 patients with+++or++HER-2 result by immunohistochemical test had HER-2 gene amplification in FISH test,while 20.00%of the patients with+result had HER-2 gene amplification in FISH test.Consistency test results of FISH and IHC showed good consistency(kappa value=0.543,P<0.05).Conclusion Immunohistochemistry and fluorescence in situ hybridization are effective in detecting HER-2 gene,and both have good consistency,which can be popularized in clinical application.
