摘要
目的观察化湿降浊中药辅助血液透析治疗慢性肾衰竭疗效及对营养状态、胃肠激素水平的影响。方法将160例慢性肾衰竭患者随机分为2组,对照组65例给予血液透析治疗,观察组65例在此基础上加用化湿降浊中药治疗,观察2组治疗前后中医证候积分、肾脏功能指标、SF-36量表评分、营养状态指标水平及胃肠激素水平变化情况,统计2组近期疗效。结果 2组治疗后面色萎黄、大便溏薄、食欲不振、神疲气短、舌淡胖及脉细弱积分均显著低于治疗前(P均<0.05),且观察组治疗后各项积分改善情况均显著优于对照组(P均<0.05);2组治疗后肾脏功能指标、胃泌素(GAS)及瘦素(Leptin)水平均显著降低(P均<0.05),SF-36量表评分、营养状态指标及生长抑素(SS)水平均显著提高(P均<0.05),且观察组治疗后各项指标改善情况均显著优于对照组(P均<0.05);观察组近期治疗总有效率显著高于对照组(P<0.05)。结论化湿降浊中药辅助血液透析治疗慢性肾衰竭可有效减轻临床症状体征,改善肾脏功能和营养状态,提高日常生活质量,调节胃肠激素水平。
Objective It is to observe the curative effect of Dissipating Dampness and Purging Turbidity medicine( DDPT) assisted hemodialysis in the treatment of chronic renal failure and its influence on the nutritional status and gastrointestinal hormone level. Methods 160 patients with chronic renal failure were randomly divided into 2 groups. 65 cases in the control group were treated with hemodialysis,65 cases in the observation group were treated with DDPT medicine assisted hemodialysis. The changes of TCM syndrome score,renal function index,SF-36 scale score,nutritional status index and gastrointestinal hormone level were observed before and after treatment in the 2 groups. The short-term effects of the 2 groups were analyzed. Results The scores of Sallow Complexion,Loose Stools,Loss of Appetite,Fatigue and Shortness of Breath,Pale Tongue and Pulse Points in the 2 groups after treatment were significantly lower than those before treatment( all P < 0. 05),and the improvement of the scores in the observation group was significantly better than that of the control group( P < 0. 05). The renal function index,GAS and Leptin levels were decreased significantly( all P < 0. 05),and the score of SF-36,the index of nutritional status and the level of somatostatin( SS) were significantly increased after treatment in the 2 groups( all P < 0. 05),and the improvement of all indexes after treatment in the observation group was significantly better than that of the control group( P < 0. 05). The total effective rate of treatment in the observation group was significantly higher than that of the control group( P < 0. 05). Conclusion The treatment of chronic renal failure with DDPT assisted hemodialysis can effectively reduce the clinical symptoms and signs,improve the renal function and nutritional status,develop the quality of daily life,regulate the level of gastrointestinal hormone.
