摘要
目的:探究四逆散合二陈汤联合莫沙必利治疗反流性食管炎的疗效的及对胃分泌功能、血清人生长素释放多肽(Ghrelin)、瘦素(Leptin)水平的影响。方法:将2019.12~2020.12期间于我院就诊的84例反流性食管炎患者随机分为观察组和对照组42例,对照组实施常规莫沙必利常规西药治疗方案,观察组在对照组基础上予以四逆散合二陈汤治疗,比较治疗8w后两组中医证候疗效差异,比较治疗前及治疗8w后两组中医症状积分、胃分泌功能[胃动素(MTL)、促胃泌素(GAS)、血管活性肠肽(VIP)]、血清学指标(Ghrelin、Leptin)水平、生活质量[胃肠道疾病患者生活质量指数(GIQLI)评分]变化。结果:治疗8w后,观察组总有效率显著高于对照组(P<0.05);两组中医症状积分、Leptin水平均较治疗前显著降低,且观察组明显低于同一时间对照组(P<0.05);两组MTL、GAS、VIP、Ghrelin水平及GIQLI评分均较治疗前显著升高,且观察组显著高于对照组(P<0.05)。结论:四逆散合二陈汤联合莫沙必利治疗反流性食管炎的疗效显著,有助于促进患者胃分泌功能改善以及血清Ghrelin、Leptin水平调节,亦有利于生活质量提高,有较高临床应用价值。
Objective:To explore the efficacy of using Sini Powder and Erchen Decoction combined with mosapride in the treatment of reflux esophagitis and its influence on gastric secretory function and levels of serum human growth hormone releasing polypeptide(Ghrelin)and Leptin.Methods:84 patients with reflux esophagitis who were treated in the hospital between December 2019 and December 2020 were randomly divided into an observation group and a controlled group,with 42 cases in each group.The controlled group received conventional western medicine of mosapride,and the observation group was treated with Sini Powder and Erchen Decoction on the basis of the controlled group.The efficacy of TCM syndromes of the two groups was compared after 8 w of treatment,and the scores of TCM symptoms,gastric secretory function[motilin(MTL),gastrin(GAS),vasoactive intestinal peptide(VIP)],serological indicators(Ghrelin,Leptin)and quality of life[Gastrointestinal Quality of Life Index(GIQLI)]were compared before treatment and after 8 w of treatment.Results:After 8 w of treatment,the total effective rate of the observation group was significantly higher than that of the controlled group(P<0.05).The TCM symptoms scores and Leptin level of the two groups were significantly decreased compared with those before treatment,and the scores and level of the observation group were significantly lower than those in the controlled group at the same time point(P<0.05).The levels of MTL,GAS,VIP and Ghrelin and GIQLI scores in the two groups were significantly increased compared to before treatment,and the levels and scores of the observation group were significantly higher than those of the controlled group(P<0.05).Conclusion:Sini Powder and Erchen Decoction combined with mosapride has a significant efficacy in the treatment of reflux esophagitis.It is helpful to improve the gastric secretory function and regulate the levels of serum Ghrelin and Leptin,and it is also conducive to the improvement of quality of life,with a high clinical application value.
